CHANGE ORDER ADDENDUM NO. 1
TO
ClientTrack'Software as a Service (SaaS)Subscription Agreement
This Change Order dated 2/27/2014,is an Addendum to the ClientTrack"'Software as a Service(SaaS)
Subscription Agreement(the "Agreement")with the original Effective Date of 2/1/2014,includes all
prior addenda and exhibits between ClientTrack,Inc. ("ClientTrack")and Weld County Department of
Human Services("Licensee").The following terms modify and are hereby incorporated as part of the
Agreement.
ClientTrack&Licensee agree that the following Contract Terms shall be amended as follows:
1.Contract Deliverable or Term shall be MODIFIED as described below.
Deliverable Definition of Scope:This scope of work includes eight(8)months of Continuous
Improvement Service in year one throughout 2014. In subsequent years two and three this scope
includes either 1.6 hours per month for 12 months or 5 months utilizing 4 hours per month chosen at
Weld County discretion.At the end of year one or year two Weld County may elect to switch to
Continuous Education Service at then current price to receive training on ClientTrack software
Administration or purchase additional hours of the Continuous Improvement Service up to 8 hours per
month.
Price
There are NO pricing modifications that affect the contract or the term of the agreement,and this
amendment is for clarification purposes only. Weld County may elect to purchase more services or
switch to Continuous Education Service at any point the County elects to train personnel on ClientTrack
Administration. Prices will prevail at the going rate at that time.
All terms,conditions, pricing and other provisions(including Exhibits)of the Agreement not modified by
this Addendum remain in full force and effect.
