This project worksheet addresses damages to CR 20.5 between CR 5 and CR 7, in Section 16,T2N, R68W. On November 14,2013, Kathleen Ruvarac, FEMA TAC Bridge Specialist, David Ray, FEMA Hazard Mitigation Specialist, Gary Moore, FEMA Environmental, Historical, Preservation Specialist, and Donald Dunker,Weld County Public Works Engineer,performed a site investigation at this location to document damages. The road was open and repairs completed at the time of inspection. One asphalt patch on roadway measured 362 ft In length x 13 ft in width=4,706 SF x 0.75 ft in depth= 3,529.50 CF x 146 LB/CF x 1 ton/2000 LB=257.7 TNS asphalt A second asphalt patch on roadway measured 19 ft in length x 13 ft in width x 0.75 ft in depth =185.25 CF x 146 L8/CF x 1 ton/2000 LB=13.5 TNS asphalt Total: 271.2 TNS asphalt Embankment Material lost in roadway void in the westbound travel lane and on north shoulder measuring 362 ft in length x 15 ft in width x 6 ft in depth=1,206.7 CY and 19 ft in length x 15 ft in width x 6 ft in depth=63.3 CY,totaling 1,270 CY. 16 LF of 18"CMP washed away from the site and was not recovered. The Applicant has provided pictures of the facility dated 9-16-2013 documenting damages. These photos are attached to this project worksheet. Total Fill Materials=975.07 CY+303.4 CY=1278.5 CY The project specialist compared the repair cost for labor and construction of similar roadway repairs with the bid and subsequent invoice received from Mountain Constructors, Inc.for$86,984.48 and found the repair estimate to be reasonable. The applicant hired an environmental consulting firm to prepare application for ACOE Section 404 Nationwide permit,Tiglas Ecological Services,5015 Swainsona Drive, Loveland, CO 80537. Invoice attached to project worksheet for work performed 9-29-2013=$445.00 Total Contracts:$87,429.48 2) Force Account: The applicant utilized force account labor,equipment and materials to install (289.16 tons)of asphaltic material on roadway. Labor-$6,707.19 Equipment-$5,546.50 Materials-$11,277.24 The project specialist compared the repair cost for construction of similar CDOT roadway repairs with the Applicant's force account labor,equipment and materials found the repair estimate to be reasonable. The applicant has provided payroll data for employees as an hourly rate including fringe benefits. Project Notes 1. Following the September 2013 flooding,applicant used contract services and/or force account resources to re-open roads/bridges and repair damage as soon as possible. At several damage sites, repairs appear to have returned the facility to its pre-disaster design,function and capacity with no known remaining work to be completed. However at other sites due to the onset of winter conditions, applicant was unable to complete the restoration,thus leaving work to be completed. Finally,at some sites,applicant has expressed concern that unanticipated work may be still be required as a result of conditions beyond their control,such as possible subsequent settlement resulting from saturated soil conditions following the flood or where high water levels or erosion/deposition prevented a complete damage assessment. This PW addresses all validated flood related damages at the specified site, including completed work and work to be completed (which damages were known to the applicant and reported to FEMA by the effective date of this PW). In the event that applicant identifies subsequent,currently unknown flood related damage at such site,applicant is advised to contact the State of Colorado Office of Emergency Management to report the damage and request an inspection prior to its repair. In the event that the State validates the damage as a direct result of the September 2013 flooding,the State will request FEMA prepare a version of this PW to address the eligible cost of repair. 3. Pursuant to 44 CFR 13.42, Grantees and Subgrantees are required to retain records, including source documentation, to support expenditures/costs incurred against the grant award,for 3 years from the date of submission to FEMA of the final Financial Status Report. 4.The applicant must obtain all required federal,state,and local permits prior to the commencement of work. 5.The subgrantee is requesting direct administrative costs that are directly chargeable to this specific project. Associated eligible work is related to administration of the PA project only and in accordance with 44 CFR 13.22. These costs are treated consistently and uniformly as direct costs in all federal awards and other subgrantee activities and are not included in any approved indirect costs. 6. Project was reviewed for 406 Hazard Mitigation and a copy of 406 HMP proposal is attached with this project. 7.The applicant is aware that all projects are subject to an insurance review as stated in 44 C.F.R. Sections 206.252 and 206.253. If applicable, an insurance determination will be made either as anticipated proceeds or actual proceeds in accordance with the applicant's insurance policy that may affect the total amount of the project. 8. 90%sampling of the timesheets and equipment logs have been reviewed and found to be complete by the FEMA project specialist. A copy of the applicant's payroll policy and example timesheets and equipment logs are attached as supporting documentation. 9. 100%of all received invoices,and cancelled checks(or other proofs of payment) have been reviewed and are attached as supporting documentation. 10.The applicant is required to adhere to State and Federal Government Procurement rules and regulations and maintain adequate records to support the basis for all purchasing of goods and materials and contracting services for projects approved under the Public Assistance program,as stated in 44 CFR 13.36.The applicant has advised they have followed their normal procurement procedures.
