DATE CATEGORY WELCO34 123-99123- 03-26-2014 B FEMA 4145 - DR -CO 00 APPLICANT:WELD(COUNTY) WORK COMPLETE AS OF: 12-05-2013: 100% Site 1 of 1 DAMAGED FACILITY: COUNTY: Weld CR 53 Bridge 53/58A LOCATION: LATITUDE: LONGITUDE: 40.410797 -104.56369 Current Version: County Rd 53 at Bridge 53/58A DAMAGE DESCRIPTION AND DIMENSIONS: Current Version: A major disaster,DR-4145-CO,with a disaster period of September 11 to September 30,was declared on September 13,2013.During the incident period of September 11 and September 30,2013,torrential rains resulted in raging torrents across Weld County,spilling flood water from the South Platte River and nearby streams,creeks and reservoirs,washed debris down rivers,inundating bridges, jamming debris at supports,and abutments,throughout the County.Damaged facility with debris is on CR53 at a county maintained bridge on an FHWA road. Bridge#53 58 A on County Road 53 North of CR58 CPS: 40.410797,-104,563690 Along County Road 53 at Bridge 53 58A,floodwaters at the South Platte River deposited 2100 CYs(758 x 448 x 178=56,100/27=2077 rounded to 2100)of vegetative debris,along the bridge,on the County Road right of way. Debris was deposited along the bridge supports threatening the structure. The debris was a hazard to the bridge facility,contributing to site permanent roadway damage,as a jam creating extreme lateral pressure on the abutment(See photo 1 &2).This debris is included in this project work sheet as Category B damage under Guidance 3(See attachment 4,Guidance 3 Memo). SCOPE OF WORK: Current Version: Scope of Work This project is part of an FHWA project. Roger Jones, Infrastructure Branch Director 4145 DR CO has determined that Cis appropriate to split out a line item for removal of debris from a project funded by the FHWA for permanent road repair and fund the removal of debris under public assistance funding. Applicant hired Kehn Construction(See attachment 1,Contract with Kehn Construction),to perform permanent repairs at the bridge,with debris removal in this PW scope of work,completed contiguous to an area of permanent work.Debris removal was accomplished incidental to permanent work(See attachment 4,Guidance 3)included in this Category B project work sheet. Contractor,making permanent repairs at the site removed 2100 CYs of vegetative debris(See photo 3).Debris was removed from the bridge and river bank not requiring equipment in the waterway.Contractor utilized equipment to remove debris and place it in dump trucks hauling it to private property at 27125 cr 12, Greeley, CO 80631,GPS 40.417178,-104.514258(See attachment 2, Letter From Property Owner&Attachment 6, Right of Entry ROE with Hold Harmless),free of charge. Debris will be recycled for use by the property owner as firewood. Debris was removed by the contractor at a cost of$12 per CY with a total cost of$25,200(2100CY x$12_$25,200)for 2100CYs of material. Debris CY price includes disposal.Total Contract Cost is$25,200(See attachments 3, Kehn Final Quantities). Scope Note: The total project amount for permanent repairs as presented to FHWA,for financing in their program,with the$25,200 included in this project work sheet subtracted, is$513,036.83. The total invoice is included as an attachment to this project work sheet(See attachment 3, Kehn Final Invoice). Direct Administrative Costs: The sub grantee is requesting reimbursement for Direct Administrative Costs(DAC)directly chargeable to this specific project.Associated eligible work is related administration of the PA project only and in accordance with 44 CFR 13.22. These costs are treated consistently and uniformly as direct costs in all federal awards and other sub grantee activities and are not included in any approved indirect cost rates.At the time this project work sheet was completed the applicant had not presented DAC costs. Record Retention: Complete records and cost documents for all approved work must be maintained for at least 3 years from the date the last project was completed or from the date final payment was received,whichever is later. Applicant is responsible for retention of all documentation associated with this project. Supporting Documentation: 100%of the documentation to support this project has been reviewed and verified by the Applicant and Project Specialist for eligibility and correctness. Procurement: The Applicant is required to adhere to Weld County procurement policy and maintain adequate records to support the basis for all purchasing of goods and materials and contracting services for projects approved under the Public Assistance program,as stated in 44 CFR 13.36.Applicant received bids,as indicated in the contract agreement(See attachment 1,Contract With Kehn Construction),for this project and has maintained a record of bids received. Monitoring: The applicant self-monitored debris removal operations by utilizing force account labor. Hazard Mitigation: This project work sheet is emergency work and no hazard mitigation opportunities have been identified.
