It is our desire to provide you with a quality solution to your environmental needs. As a highly qualified

ARC Abatement, Inc. is pleased to provide you with this proposal for the following SCOPE of WORK.
Remove and dispose of approximately 1,915 square feet of asbestos containing (ACM) transite siding from
using proper wet
located at 23459 CR 58 in Greeley, CO. Work will be completedp pPPE,

Specialist (AMS)is included in the pricing below
PRICE and DIJRATION•
ARC proposes to perform the Scope of Work for the amount of: Eight thousand seven hundred ninety one
dollars;(8,791.00)
Based upon our experience the estimated number of working days to perform the Scope of Work will be:4
working days.
PROPOSAL TERMS:
1. Includes costs for: Labor,Material and Applicable Taxes.
2. Compliance with federal,state and local laws governing abatement.
3.Site Conditions being reasonably similar to those at the time of inspection.
4.Reasonable interpretation of existing conditions based upon Owner provided information of the work area.
5. No contingency or consideration given for delays to the work.

6.Air monitoring, for our workforce, as per Federal,State and Local regulations is included in our price.
7. Final Clearances will be provided by third parry Consultants and or in close out documentation.
8. Pricing based on normal working hours unless otherwise indicated.
9. Single mobilization to the site unless otherwise indicated. (Additional mobilizations to be priced
separately)
10. Owner provides adequate electricity, potable water, on-site parking and sufficient storage space for
materials and equipment. Utility connections to be adjacent to work area and compatible with ARC
requirements.
11.All non-stationary items are to be removed by others prior to the start of our work
12. ARC will not be liable for damage to items left within the specified work area.
13. All IIVAC systems within the containment areas must be shut down and locked out by Owner or
Owners representative.
14. Proposal based upon mutually agreeable contract terms and conditions.
15. ARC will employ all reasonable efforts to minimize damage to surfaces and work areas in the
construction of regulated containments, but will not be held liable for any damages due to the construction
of regulated containments needed to perform the environmental services as noted within this proposal.
16. Client agrees that ARC has been given the privilege by the property Owner to enter the premises,
perform services, use of any blue prints, etc., and agree to hold harmless, and defend ARC Abatement, its
Officers, Employees, Sub-Contractors, for any and all claims, costs or damages that result from services
contracted, in the absence of gross neglect or willful wrongdoing of ARC. ARC is expressly authorized to
sign any required disposal forms on behalf of the Owner or generator of any waste removed from the site.
17.Payment and Performance Bond costs are not in this proposal.
18. Unknown conditions may exist at the site which,were not evident at the time of site assessment. The scope
of work is specific, and does not include contingencies for unknown conditions. Additional work, if required,
will be priced separately.
19.This proposal may be withdrawn for any reason if not accepted within Sixty (60) days.
20. AlL INVOICES arc due upon receipt and are expected to be paid within 30 days.

Project Manager Waiver
Colorado Regulation No. 8, Part B, requires that project managers be used to monitor asbestos abatement projects in which
the amount of friable asbestos containing material exceeds 1000 linear feet or 3000 square feet. This requirement may be
waived if the contractor performing the abatement has a history of compliance with Regulation No. 8, or if the'building owner
can demonstrate that this requirement is overly burdensome or not feasible by providing a written explanation below.
Check which condition applies:
X The abatement contractor has informed me that they have fewer than two compliance determinations with a finding
of guilty in the preceding two years.
For the above project,the project manager requirement is overly burdensome op not feasible. I request a waiver
from the project manager requirement for the following reason(s). Please attach additional pages if necessary.
I understand that use of a Project Manager is an effective means of ensuring the project is properly conducted in compliance
with applicable regulations.

RESOLUTION
RE: ACTION OF BOARD CONCERNING WAIVER OF TEN-DAY WAITING PERIOD FOR
CONSIDERATION AND INVESTIGATION OF BID #B1400158 - DEPARTMENT OF
BUILDINGS AND GROUNDS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Department of Purchasing has requested waiver of the ten-day waiting
period for Bid Request #61400158, Asbestos Removal/Abatement — Parkway House
Demolition, for the Department of Buildings and Grounds, due to the discovery of asbestos in
the demolition of the Parkway House, and in order for the Parkway road project to remain on
schedule, and
WHEREAS, the Board of County Commissioners deems it advisable to waive the
ten-day waiting period for said bid.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the ten-day waiting period for Bid Request #61400158, Asbestos
Removal/Abatement — Parkway House Demolition, for the Department of Buildings and
Grounds, be, and hereby is, waived.
BE IT FURTHER RESOLVED by the Board that the low bid from ARC Abatement be
accepted, in the amount of $21,551.00, be, and hereby is, accepted.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of August, A.D., 2014.

Subject: Asbestos Removal/Abatement-Parkway House Demolition; Bid#B1400158
As advertised, this bid is to remove/abate the asbestos that was discovered in the house which is due to
be demolished. The low bid received was from ARC Abatement and meets specifications. Therefore,
Buildings& Grounds is recommending the bid be awarded to ARC Abatement for$21,551.00.
Since the house is scheduled to be demolished starting September 15th, Buildings and Grounds is
requesting emergency approval in order for the Parkway road project to remain on schedule.
