RE: APPROVE SECOND AMENDMENT TO AGREEMENT FOR INMATE HEALTH
SERVICES AND AUTHORIZE CHAIR TO SIGN - CORRECT CARE SOLUTIONS, LLC
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Second Amendment to Agreement for
Inmate Health Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Sheriff's Office, and Correct Care
Solutions, LLC, commencing January 1, 2015, and ending December 31, 2015, with further terms
and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Second Amendment to Agreement for Inmate Health Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the on behalf of the Sheriff's Office, and Correct
Care Solutions, LLC, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of December, A.D., 2014.

SECOND AMENDMENT TO AGREEMENT FOR INMATE HEALTH SERVICES
This Second Amendment(the"Amendment")to the Agreement for Inmate Health Services
by and between the County of Weld, Colorado ("Weld County") and Correct Care Solutions, LLC
("CCS")of original date November 30, 2012 and as amended January 1, 2014 (the"Agreement"),
is effective as of January 1, 2015.
WHEREAS, the Agreement may be amended pursuant to its terms as set forth in Article
XI (A) by written consent of the parties; and
WHEREAS, Weld County consents to an increase in compensation paid to CCS pursuant
to Article VI(A)(2) of the Agreement to accommodate the rising cost of healthcare.
NOW, THEREFORE, in accordance with the terms for modification of the Agreement and
for good and valuable consideration,the receipt and sufficiency of which is hereby acknowledged,
effective January 1, 2015 Article VI(A)(1) shall be deleted and amended as follows:
1. The monthly amount to be paid by Weld County to CCS under this Agreement is
$273,552.19(Two Hundred Seventy-Three Thousand Five Hundred Fifty-Two Dollars and
Nineteen Cents) for the period beginning January 1, 2015 through December 31, 2015.
All terms and conditions of the Agreement not expressly amended herein will remain in
full force and effect.
IN WITNESS WHEREOF, Weld County and CCS have executed this Amendment on the
most recent date indicated below.
