RE: APPROVE FOUR PURCHASE OF SERVICES AGREEMENTS FOR SENIOR CENTER
OPERATIONAL ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with four Purchase of Services Agreements for
Senior Center Operational Activities between the County of Weld,State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Area Agency on Agency, and the various Senior Centers, as follows, commencing
January 1, 2014, and ending June 30, 2014, with further terms and conditions being as stated in
said agreements, and
Hill N Park Senior Citizen Program
Lochbuie After Glows Senior Citizens Center
Nunn Senior Center
Pierce Senior Citizens, Inc.
WHEREAS,after review,the Board deems it advisable to approve said agreements,copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the four Purchase of Services Agreements for Senior Center Operational
Activities between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Agency, and the various Senior Centers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreements.

RE: Purchase of Services Agreements beten the Weld County
Department of Human Services' Area Agency on Aging and
Various Providers
Enclosed for Board approval are Purchase of Services Agreements between the Department
and Various Providers. These were reviewed under the Board's Pass-Around Memorandum
dated January 30, 2014, and approved for placement on the Board's Agenda.
Hill N Park Senior Center, Lochbuie After Glows,Nunn Senior Center and Pierce Senior
Center face financial hardship in terms of keeping their centers open due to their limited
funds. All of these sites are part of our Senior Nutrition Program and play a vital role in
senior services in their rural communities.
The centers will utilize the grants to help pay for utilities, supplies, water and other
operational expenses.
The effective date of these agreements is valid through June 30, 2014. The amount of
funding for each is as follows:
• Hill and Park Senior Center$375
• Lochbuie AfterGlows Senior Center $375
• Nunn Senior Center - $375
• Pierce Senior Center$375

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this I st day of January 1, 2014 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of
Human Services'Area Agency on Aging, hereinafter referred to as "AAA" and the Hill &Park Senior Citizen, Inc
hereinafter referred to as"Hill &Park".
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS, "AAA" considers small rural senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Hill &Park Senior Citizens,Inc" is a senior center providing recreational,nutritional, and educational
opportunities for senior citizens;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies(Older Coloradoan Fund) in the sum of$375.00 to"Hill &Park."
2. "Hill &Park" agree to expend the grant monies to pay for operating expenses for the center to include: electricity,
gas,telephone, insurance, office supplies, postage, and other minor operating expenses.
3. "Hill &Park" agree to maintain documentation of the above expenditures and to seek prior approval from the AAA
if the expense source is not listed in the statement above.
4. "Hill &Park" understand that terms of this agreement are valid through June 30,2014.
IN WITNESS WHEREOF,the parties hereunto have caused this Agreement to be duly executed as of the day and year first
hereinabove set forth.
