RE: APPROVE CHILD PROTECTION AGREEMENTS FOR SERVICES AND AUTHORIZE
CHAIR TO SIGN -GREELEY COUNSELING CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,on January 20,2014,the Board was presented with an Agreements for Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the Greeley
Counseling Center, with terms and conditions being as stated in said agreement, at which time the
Board deemed it advisable to continue the matter to January 22, 2014, and
WHEREAS,after review,the Board deems it advisable to approve said agreement, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Child Protection Agreement for Services between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County,on behalf of
the Department of Human Services, and the Greeley Counseling Center, be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foreaning Resolution was, on motion duly made and seconded, adopted by
the following vote on the 22ndday of January, A.D., 2014, nunc pro tunc December 9, 2013.

SUBJECT: Amendment to the Child Protection Agreement for
Services between the Weld County Department of Human
Services and Greeley Counseling Center
Enclosed for Board approval is a Child Protection Agreement for Services between the Department
and Greeley Counseling Center. This Agreement was reviewed under the Board's Pass-Around
Memorandum dated December 19, 2013, and approved for placement on the Board's Agenda.
This amendment to the Greeley Counseling Center (Jack Gardner) is due to a case requiring
travel outside of the Greeley Metro area to conduct an Interactional evaluation. In order to
accommodate the required travel, a travel surcharge of$60.00/Episode (Round-trip travel
surcharge per occurrence) is being added to the agreement.
No other changes are requested at this time.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
GREELEY COUNSELING CENTER
This Amendment,made and entered into the 9's day of December 2013 by and between the Board
of County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter
referred to as"Department,"and Greeley Counseling Center, 1228 8th Street,Greeley,CO 80631,
hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Mental Health Services(PY-
13-14-CORE-0139)in order to add a travel surcharge under this agreement;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Item 2,Fees for Service,is hereby revised to include the following:
$60.00/Episode(Round-trip travel surcharge per occurrence.)
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,
month, and year first above written.
