RE: APPROVE WORK ORDER FORM (FLOOD REPAIRS - EM-BR16.5-1B) PURSUANT
TO AGREEMENT FOR DESIGN SERVICES AND AUTHORIZE CHAIR TO SIGN -ICON
ENGINEERING, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
EM-BR16.5-1 B) Pursuant to Agreement for Design Services between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Public Works, and Icon Engineering, Inc., commencing January 13, 2014, with a
completion date of March 30, 2014, and with further terms and conditions being as stated in said
work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - EM-BR16.5-1B) Pursuant to
Agreement for Design Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Public
Works, and Icon Engineering, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said work order form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of February, A.D., 2014, nunc pro tunc January 13, 2014.

Weld County 2013 Flood Recovery
EM-BR16.5-1B-CR16.5 Bridge Over Boulder Creek
February 11, 2014
Structure Description : This two span bridge is 127-feet long with a driving surface width of 31.5-feet.
The bridge deck has an asphalt driving surface placed directly on four precast concrete double tees
girders. Construction drawings of the bridge
indicate that the abutments and the center
pier have concrete spread-footings. There is - -i._ . -• - �,____ _—

Each corner of the bridge has a 45 degree ie* • ;
concrete wingwall. The bridge was originally T :; •-tl.-.-., +44 �-
built in 1983 . The 2012 Inspection Report , y 4 .
indicates a sufficiency rating of 96.9 with :_ r --� ,
substandard bridge rails. This two lane .k..... „,.- :r..
roadway was estimated to have an average
daily traffic load of 201 . • -
This bridge withstood the 2013 flood event in that
it was put back into service without structural
repairs. However, the roadway, particularly to the _
west, was significantly damaged . At the time of _ -

our site visit, there was no obvious indication that

however the damage to the road and signs of
r debris deposits certainly indicate that the depth of
- _ water exceeded the bottom of the pier caps.

bridge has not been determined. It is our opinion that riprap should be placed at each abutment and at
P P P
the center pier. Additional cleanup of the channel will be required prior to placement of riprap. A
detailed inspection of the bridge should be conducted and a hydraulic analysis completed in order to
confirm that the riprap along the abutment will not create a floodplain rise. As noted above, the bridge
showed signs of debris on the abutment bearing surface prior to the flood event, which may be an
indication that it is hydraulically undersized. The picture to the left (provided by the County) is of the
roadway west of the bridge and shows debris build-up on the upstream fence. Damage to the roadway
consisted of washing out the downstream shoulder, although the majority of the gravel surface roadway
was left intact. Placement of TRM along this reach is problematic due to maintenance activities for the
gravel shoulder. At a minimum, exposed bare soil should be revegetated.
Work Required To Return Assets To Pre-Flood Conditions: Assuming that the pre-spring runoff repairs
are made, and assuming that the detailed inspection does not reveal scour damage or other structural
damage, we do not envision further modifications will be required to bring the assets up to pre-flood
conditions.
Work Required For Mitigation or Betterment: Depending on the results of the hydraulic analysis, this
bridge may need supplemental hydraulic capacity. The recommendations from the previous 2012
inspection report are still valid, consisting of conducting maintenance activities including installing
adequate bridge and approach railings to meet current standards and to clean and seal open joints
Right Of Way Needs: A construction access easements should be obtained on both sides of the bridge
to allow repairs to the erosion in these areas, and along the roadway's northern side west of the bridge.
Provision for a construction staging area will be required. Discussion will be held with the County to
determine if these easements should be temporary or permanent in order to provide future
maintenance and or construction of new bridge. We anticipate that the roadway repairs will be limited
to County ROW. In the event that the mitigation / betterment repairs are initiated, additional
temporary access easements may be needed to if a temporary roadway by-pass is required to keep the
roadway open to traffic during construction of a new bridge.
Construction sequencing, constructability issues: We do not foresee significant sequencing or
constructability issues for this work. It will be necessary to schedule construction and sequencing in
order to allow the construction contractor to perform dewatering operations as the erosion damage at
the west side of the north abutment may extend down to the water's edge. If the flows in river are too
high to dewater effectively we may have to wait to complete construction on this site.
Detailed Scope of Work: We envision our work to follow the general outline below.
Pre-Design Tasks:
1. Meetings: Common to all projects will be attending weekly meetings either as a participant with
the other three consultants, or as a part of the projects specifically assigned to ICON. Meetings
with the other consultants are assumed to be held at the Weld County Public Works office,
while the ICON specific meetings are assumed to be held at the Weld County Services Center
located near the County Road 24 Exit from I-25. These meetings are assumed to be held starting
on January 28th and running through the end of the first phase of design.

2. Obtain Base Mapping: We will prepare base mapping using the available FEMA LiDAR
information flown after the flood event. Additional field surveys will be required to pinpoint
critical elevations and locations of improvements.
3. We have assumed that construction easements from two properties will be required, and will
prepare exhibits, descriptions and a ROW drawing.
4. Utility Research: We will request utility maps from owners known to operate in the area, and
will plot the locations based upon the provided information. This information will be further
refined by field surveys of exposed lines. We have included potholing services to locate 3-
utilities.
5. Geotechnical Investigations: We do not anticipate performing geotechnical investigations as a
part of this phase of work.
6. Environmental Permitting Evaluations: ERO will prepare a listing and opinion of need for all/any
environmental permits that may be required for each phase of work. Accompanying this
subtask will be confirming letters from applicable review entities. (Note: it is assumed that no
Individual 404 Permit is required).
Phase 1: Design Work To Be Done So Construction Can Be Completed Before Spring Runoff:
1. Detailed site visit
2. Prepare Hydraulic Analysis of proposed improvements (Note: We assume that a no rise situation
will result,therefore no FEMA LOMC required).
3. Prepare plans for proposed improvements (described herein)
4. Depending on results of the above tasks, make recommendations for future phases of work
5. Prepare plans for riprap placement at piers and abutment
6. Prepare revegetation plans for the roadways as determined compatible with gravel roadway
maintenance activities.
7. Coordination with local land owners
8. Coordination with ICON Team Members
9. Quantity Estimates
10. Unit Price Cost Estimate
11. Prepare Write-up Bid Documents
12. Submit Draft Plans and Specifications To County
13. Revise and resubmit
14. Provide up to 50-hours of Construction Administration/Observation
Phase 2: Design Work To Return To Pre-Flood Conditions Note: the scope of work required to
design the work needed to return the site to pre-flood conditions will be covered under another
work order. At this time, no work contemplated under this phase
Phase 3: Design Work For Mitigation / Betterment Work Note:the scope of work required to design
the work needed for mitigation / betterment will be covered under another work order. At this
time, we foresee it possibly including:
1. Design of a replacement bridge and or adding supplemental hydraulic capacity.

The receipt of BT Construction,Inc. of a signed copy of this Proposal/Contract constitutes acceptance of this Quotation,subject to and
including all terms and conditions as shown.
The entire proposal is subject to acceptance within thirty(30)days of this date.
NOTE:Please note us as soon as possible if you intend to award this work to BTrenchless.Any alteration or deviation from the above
proposal will be executed only upon written request and will therefore become cost in addition to the above estimate.Any deviation
from the scope of work will be completed following the acceptance of a change order.BTrenchless hereby warrants for a period of one
