Wallace, MD, MPH, Executive Director Department of Public Health & Environment DATE: January 23, 2014 SUBJECT: Agreement for Professional Services with Jessica Alicdan Enclosed for Board review and approval is a contract between the Weld County Department of Public Health and Environment and Jessica Alicdan. The Health Department needs to process surveys received from the 2013 Weld County Community Health Survey. This scientific survey is conducted by the Health Department every three years and is required to inform the Department on conducting its essential public health functions. The survey asks residents about their physical and mental health, health insurance, and health behaviors. There are also questions about environmental health issues, the effects of the September flood, and two open-ended questions. The Health Department does not currently employ enough qualified data entry personnel. A temporary data entry assistant is needed to enter approximately 500 health surveys with a high degree of accuracy and speed by the end of first quarter 2014. Approximately 1,000 paper- based surveys have been received so far and another 500 are expected to be received in January. These surveys contain over 200 numeric fields and written text. Currently about 500 surveys have been entered by existing qualified staff and a student intern. About 700 surveys have been received online and do not require entry into a database. Funding for this contract will not exceed $2,000.00 through March 31, 2014. I recommend approval of this Professional Services Agreement with Jessica Alicdan . iv-6 /740YMa.WealthO Ubl C. 2014-0242 1-a7- 14 da_ pit pout (-A OtH AGREEMENT FOR PROFESSIONAL SERVICES THIS AGREEMENT is made by and between the County of Weld, State of Colorado, whose address is 1150 O Street, Greeley, Colorado, 80631 ("County"), and Jessica Alicdan_ whose address is 2400 W.
