HomeMy WebLinkAbout20140367.tiff RESOLUTION RE: APPROVE AMENDMENT #1 TO PROFESSIONAL PROVIDER AGREEMENT FOR ACCOUNTABLE CARE COLLABORATIVE PROGRAM AND AUTHORIZE CHAIR TO SIGN - COLORADO ACCESS WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Amendment #1 to the Professional Provider Agreement for the Accountable Care Collaborative Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and Colorado Access, for the delegation of care management to North Colorado Health Alliance, commencing upon full execution, with terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Amendment #1 to the Professional Provider Agreement for the Accountable Care Collaborative Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and Colorado Access be, and hereby is, approved. Wallace, MD, MPH, Executive Director Department of Public Health & Environment DATE: January 31, 2014 SUBJECT: First Amendment Professional Provider Agreement for Accountable Care Collaborative Program Enclosed for Board review and approval is a contract between the Weld County Department of Public Health and Environment and Colorado Access. The Weld County Department of Public Health and Environment participates in the Medicaid Accountable Care Collaborative in Region 2 through Colorado Access. The Department receives a per member per month payment to provide health care home services and agrees through a contract with Colorado Access to have local care management provided through a sub- agreement with the North Colorado Health Alliance. Colorado Access prefers that care management be delivered locally rather than remotely from Denver. This amendment authorizes Colorado Access to delegate care management to the North Colorado Health Alliance. This local relationship has worked well and the Department desires to maintain this local relationship through this delegation agreement amendment. The relationship between the Department and the Region 2 accountable care collaborative has been enhanced through this local care management delegation agreement with Colorado Access. The Department has a close working relationship with the North Colorado Health Alliance care managers. Keeping care management local is important to the Department and the patients we serve. There is no financial obligation to the Department to delegate care management to the North Colorado Health Alliance. It is the Departments preference to continue with the agreement. I recommend approval of this First Amendment to the Professional Provider Agreement for Accountable Care Collaborative Program with Colorado Access. 2014-0367 BOARD OF COUNTY COMMISSIONERS PASS-AROUND REVIEW/WORK SESSION REQUEST RE: Amendment to the Professional Provider Agreement For Accountable Care Collaborative Program DEPARTMENT: PUBLIC HEALTH&ENVIRONMENT DATE: August 5, 2013 PERSON REQUESTING: Mark E. Wallace,MD, MPH, Executive Director Brief description of the problem/issue: Enclosed for Board review and approval is an amendment to the Professional Provider Agreement for Accountable Care Collaborative Program dated February 20,2013. The Weld County Department of Public Health&Environment participates in the Medicaid Accountable Care Collaborative in Region 2 through Colorado Access. The Department receives a per member per month payment to provide health care home services and agrees through a contract with Colorado Access to have local care management provided through a sub-agreement with the North Colorado Health Alliance. The amendment has been reviewed by the County Attorney's Office and support advancing the Agreement to the Board. What options exist for the Board? Should the Board elect not to support the amendment the Department would then have care management services provided directly by Colorado Access from Denver to our enrolled Medicaid beneficiaries. This would be a less effective and less desirable manner in which to deliver these services to patients. WHEREAS,Colorado Access and the State of Colorado have entered into an agreement for the Regional Collaborative Care Organization; WHEREAS,Colorado Access and Provider previously entered into that certain Professional Provider Agreement For Accountable Care Collaborative Program(the"Agreement"),dated February 20,2013; WHEREAS,Provider has entered into a Primary Care Medical Provider("PCMP")agreement with the State of Colorado; WHEREAS,Section 4 of Exhibit A of the PCMP agreement requires Provider to work with the RCCO for care coordination; WHEREAS,Colorado Access has previously entered into a Regional Collaborated Care Organization Subcontractor Agreement for Care Management with the North Colorado Health Alliance on February 1, 2012; WHEREAS,Colorado Access and Provider now wish to amend the Agreement as set forth below. NOW,THEREFORE, in consideration of the foregoing,the parties hereby agree as follows: Care Management(also referred to as case management and care coordination): 1. Provider and Colorado Access Agree that North Colorado Health Alliance will provide care management services to the Provider's Colorado Access RCCO Member patients. Termination 1. Either party may terminate this Amendment upon ninety(90)days prior written notice. Termination of this Amendment does not terminate the Agreement.
