APPLICATION
On April 17, 2014, Carrizo Oil and Gas, Inc. ("Carrizo" or "Applicant") filed a verified
application pursuant to §34-60-116 C.R.S. for an order to:
1) Approve an additional ten wells for a total of up to 16 horizontal wells within an
approximate 640-acre drilling and spacing unit established for the Application Lands.
2) Require the productive interval of the weilbore to be located no closer than 300
feet from the unit boundaries, and no closer than 300 feet from the treated interval of any other
wellbore located in the unit.
3) Applicant requests to locate the surface facilities for the proposed wells on no
more than 1 wellpad per quarter section within the unit.
PRIOR RULES AND ORDERS
(available online at: http://cogcc.state.co.us, under "ORDERS")
• On August 12, 2010, Order No. 535-1 established four approximate 640-acre drilling and
spacing units and approved up to two horizontal wells within each unit for the Niobrara
Formation. Section 26, Township 8 North, Range 61 West, 6th P.M. is subject to this
Order.
• On January 7, 2013, Order No. 535-238 approved up to six horizontal wells within each of
10 approximate 640-acre drilling and spacing units for the Niobrara Formation. Section
26, Township 8 North, Range 61 West, 6th P.M. is subject to this Order.
NOTICE IS HEREBY GIVEN, pursuant to: 1) the general jurisdiction granted to the Oil
and Gas Conservation Commission of the State of Colorado under §34-60-105 C.R.S.; 2)
specific powers granted pursuant to §34-60-106 C.R.S.; 3) the State Administrative Procedures
Act at §24-4-105 C.R.S.; and 4) the Commission's Series 500 Rules at 2 CCR 404-1, that the
Commission has scheduled the above-entitled matter for hearing on:

protest or intervention to the person filing the application. One electronic
(cogcc.hearings_unit@state.co.us), one original and two copies shall be filed with the
Commission. Anyone who files a protest or intervention must be able to participate in a
prehearing conference during the week of June 2, 2014. Pursuant to Rule 511., if the matter
is uncontested, it may be approved without a hearing, based on review of the merits of the
verified application and supporting exhibits.

Carrizo Oil and Gas, Inc. ("Applicant"), by and through its attorneys, Holland &
Hart, LLP respectfully submits this Application to the Oil and Gas Conservation
Commission of the State of Colorado (the "Commission") for an order allowing up to
16 wells in an established 640-acre drilling and spacing unit on the following described
lands:
Township 8 North, Range 61 West, 6th P.M.
Section 26: All
Weld County, Colorado (the "Application Lands").
In support of its Application, Applicant states and alleges as follows:
1. Applicant is a corporation duly authorized to conduct business in the State
of Colorado and is an operator in good standing with the Commission.
2. Applicant owns substantial leasehold interests in the Application Lands. A
well location map of the Application Lands is attached as Exhibit A.
3. On August 12, 2010, the Commission issued Order No. 535-1 which, among
other things, established four approximate 640-acre drilling and spacing units for certain
lands, and allowed up to two optional horizontal wells„-each-ofthe-units for development- -
and operation of the Niobrara Formation. Section 26, Township 8 North, Range 61 West,
6th P.M. is subject to this Order for the Niobrara Formation.
4. On January 7, 2013, the Commission issued Order No. 535-238 which,
among other things, approved up to six horizontal wells within certain lands for the
development and operation of the Niobrara Formation. Section 26, Township 8 North,
Range 61 West, 6th P.M. is subject to this Order for the Niobrara Formation.
5. Applicant requests approval to drill, at its option, an additional ten
horizontal wells, for a total of up to sixteen horizontal wells in the unit, with internal
wellbores offset by 300 feet and unit boundary setbacks of 300 feet.

6. Applicant asserts that the requested wells are necessary to prevent waste,
protect correlative rights, and to assure the maximum efficient recovery of oil, gas and
associated hydrocarbons from the Niobrara Formation on the Application Lands.
7. That in order to promote efficient drainage of the Niobrara Formation
underlying the application lands, to prevent waste, and to protect correlative rights, the
Commission should allow the drilling of an additional ten, for a total of up to sixteen
horizontal wells in said unit. The specified size and configuration of the requested
drilling unit is not smaller than the maximum area that can be economically and
efficiently drained by the requested well(s).
8. In accordance with Rules 507.b.(1) and (4), the names and addresses of
the interested parties to this Application are set forth in Exhibit B, and the undersigned
certifies that copies of this Application shall be served on each interested party as
required by Rule 503.e.
9. Applicant agrees to be bound by the oral order of the Commission.
WHEREFORE, Applicant respectfully requests that this matter be set for hearing,
notice be given as required by law and that upon such hearing this Commission enter its
order:
A. Providing that Applicant may drill, at its option, an additional ten for a total
of up to sixteen horizontal wells in the unit, with internal wellbores offset by 300 feet and
unit boundary setbacks of 300 feet.
B. For such other findings and orders as the Commission may deem proper
or advisable in the matter. .
