RE: APPROVE WORK ORDER FORM (FLOOD REPAIRS - WEL-BR54/13A) PURSUANT
TO AGREEMENT FOR DESIGN SERVICES AND AUTHORIZE CHAIR TO SIGN -
AYRES ASSOCIATES, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
WEL-BR54/13A) Pursuant to Agreement for Design Services between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Public Works, and Ayres Associates, Inc., commencing January 13, 2014, with
further terms and conditions being as stated in said work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - WEL-BR54/13A) Pursuant to
Agreement for Design Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Public
Works, and Ayres Associates, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said work order form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 3rd day of February, A.D., 2014, nunc pro tunc January 13, 2014.

EXHIBIT A- SCOPE OF WORK
Weld County 2013 Flood Recovery

Structure Description:
This two-span bridge with a sharp nosed wall pier has a length of 154.3 feet. The bridge
foundation is on driving piles below bedrock. The abutments feature wing walls at 45 degrees to
the road upstream and downstream. The 2010 bridge inspection report gave a Sufficiency
Rating of 84.5 and an Item 113 (scour critical) code of 5, which means the structure was
expected to be stable under anticipated scour conditions.

Flood Damages and Ongoing Issues:
A portion of the embankment behind the left (east) abutment (upstream side), upstream wing
wall, and the abutment riprap protection were washed out during the flood. The roadway
immediately to the west, which is substantially lower than the bridge, was overtopped with
portions of the south shoulder and embankment slope being eroded as well.

Ayres staff visited the crossing site with Richard White of Weld County on January 17, 2014.
The east approach has been repaired and the bridge is open to traffic but with a 15 ton load limit
because of the uncertainty of how much the east abutment was undermined by scour but there
is no evidence of structural deficiency or settling of the east abutment after the flood event. The
main channel has widened considerably from the pre-flood condition and the scour hole at the
east abutment is roughly 7 feet deeper than the thalweg immediately downstream.
Prior to the flood, concrete rubble revetment was present along the both sides of the abutments
and the upstream and downstream banks. There is an exposed and now floating Excel flexible
gas pipeline just downstream of the bridge. This gas pipeline was not exposed prior to the flood
event and was underneath a low water crossing that has been washed away. There was also a
low water crossing 60 feet upstream of the bridge that has also been washed away during the
flood event. On the upstream side of the bridge is a fiber optic line that crosses attached to the
bridge. This fiber optic line is exposed at the repaired abutment.
Work Recommended Prior to Spring Runoff(Phase 1):
• Place additional rock riprap at the upstream corner of the east abutment
• Place rock riprap under the bridge along the toe of the east abutment and upstream
along the left and right bank within the right of way.
• Survey the right of way limits, the bridge surface and along the embankment side slopes.
• Hydrosurvey under the bridge to determine the size and extent of the scour hole.
• Assuming no temporary easement or ROW acquisition for this construction
Work Anticipated for Phase 2:
• Re-grade and protect with TRM and rock riprap the south embankment of the roadway
west of the roadway for 600 feet.
• Remove and replace the fill behind east abutment.
• Rebuild and protect the east abutment slope.
• Protect the upstream embankment slopes of both abutments with rock riprap
• Place rock riprap on both banks to 50 feet upstream and downstream of the bridge.
Potential Phase 3 work for Betterments and/or Resiliency:
• Rebuild the upstream left bank alignment into the bridge and protect with rock riprap
revetment or rock spurs.
• Alter the upstream channel by removing the spit of land on the upstream end of the right
bank point bar which will provide a better alignment with and transition into the bridge
reach.
• Remove deposition from top of point bar and use removed soil to backfill flood return
gullies on right bank upstream of bridge.
• Construct guidebank on right bank upstream of bridge.
Detailed Scope of Work for Phase 1:
1. Kickoff Meeting. Ayres project manager/engineer and geomorphologist will meet with
County project manager at County Public Works office.
2. Site Assessment. Ayres project manager/engineer and geomorphologist meet at the site
with County project manager to observe current field conditions, assess geomorphology
and discuss what emergency repairs were done.

3. Survey. Conduct boundary survey of the right of way limits and topographic survey of the
road pavement and embankment within the right of way from 600ft west to east of the
eastern approach of the bridge with greater detail around the east abutment including
hydrosurvey of the scour hole.
4. Prepare brief environmental documentation for permitting work at this site.
5. Progress Meeting. Conduct one progress meeting with County project manager at
County Public Works office.
6. Design. Develop design of Phase 1 riprap work at the east abutment.
7. Plans and Deliverables. Prepare abbreviated Phase 1 construction documents (following
CDOT "writeup plans" format). Estimate quantities and prepare engineer's estimate of
probable cost for Phase 1 activities. Also prepare a ROW Plan for CDOT's review to
cover anticipated work through Phase 2.
