RE: APPROVE WORK ORDER FORM (FLOOD REPAIRS - EM-BR53/58A)
PURSUANT TO AGREEMENT FOR CONSTRUCTION INSPECTION SERVICES AND
AUTHORIZE CHAIR TO SIGN -ATKINS NORTH AMERICA, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
EM-BR53/58A) Pursuant to Agreement for Construction Inspection Services between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Public Works, and Atkins North America, Inc., commencing
April 1, 2014, and ending June 1, 2014, with further terms and conditions being as stated in said
work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - EM-BR53/58A) Pursuant to
Agreement for Construction Inspection Services between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Department
of Public Works, and Atkins North America, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said work order form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of March, A.D., 2014.

SCOPE OF WORK FOR
WELD COUNTY POST FLOOD CONSTRUCTION INSPECTION

I. GENERAL
Description of the project
The scope of services will include the work items necessary to perform construction management for the
project EM-BR53/58A. This will include inspection, measurement for payment, and documentation of all
work. In addition, design services during construction will be provided as needed.

II. SCOPE OF SERVICES AND COST ESTIMATE
The scope of services for the project includes construction project management, inspection,measurement
of quantities, and documentation of all work associated with the project.
The following work tasks will be completed for the preparation of one abbreviated construction plan
package for the recommended restoration activities.
General Scope of Work
The scope of work is for one Consultant Project Engineer and for design services during construction as
needed.
The Project Engineer will be responsible for the documentation and inspection of construction activities,
supervision and documentation of time and material work,and preparation of daily diaries documenting
contractor activities and relevant observations. Inspection of materials and methods of construction to verify
compliance with contract specifications,preparing pay estimates,change orders,reviewing and approving
Methods of Handling Traffic(MHT's),monitoring the project schedule and financial status,resolving material
and traffic control issues,and coordination/contact with local entities and business with respect to construction
activities.

The time period for the work described in this scope of work covers the period from approximately April 1,
2014(or upon notice to proceed)to June 1,2014.
Authorization to Proceed
Work will not commence until written Notice to Proceed is received by Atkins, and will be completed in the
time specified.
Routine Billing& Reporting
Atkins will provide monthly billing formats,suitable to Weld County, for all contract activities performed by
the Consultant.
Project Standards
All documentation and work will be in accordance with the latest versions of the Colorado Department of
Transportation's Construction Manual, Colorado Department of Transportation Inspector's Checklist, Standard
Specifications for Road and Bridge Construction, applicable M&S standards, and the construction Contract
currently in use when the construction project is advertised, revised under advertisement,and as modified by
change order,unless otherwise directed by Weld County.
Labor,Materials,Vehicles& Equipment
Atkins will furnish all personnel, equipment and transportation,including appropriate vehicles(equipped with
flashing amber beacon), cell phone, digital camera,computers,and miscellaneous equipment(calculator,office
and lab supplies, safety equipment,etc.)required for performing the work.
Design Support During Construction
Atkins will provide design support during construction as required to address changes.
Post Construction Support
Completion,Inspection and Punch List:Participate in the final inspection with the Contractor,Weld County
upon substantial completion. The result of the inspection is the development of a punch list of remaining and/or
outstanding work to be performed by the Contractor prior to final project acceptance.
As-Constructed Drawings:Complete as-constructed drawings of work completed by the Contractor including
final pay quantities.
Completion Inspection and Close-Out: Following the completion of all punch list items by the Contractor,
participate in the final inspection with representatives from the Contractor and Weld County,to confirm the
completion of all work.The result of this inspection will constitute final project acceptance.
Preparation of Final Pay Estimate: Determine final quantities with appropriate supporting documentation and
checks and prepare final pay estimate.
