RE: APPROVE ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - MONAGHAN PSYCHOTHERAPY SERVICES,
INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Assumption of Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and Monaghan Psychotherapy Services, which was previously entered into with
Counseling Services of Longmont, Inc., commencing November 1, 2013, with further terms and
conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve the assumption of said
agreement by North Range Behavioral Health, a copy of which is attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Assumption of Child Protection Agreement for Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and Monaghan Psychotherapy
Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of February, A.D., 2014, nunc pro tuns November 1, 2013.

Monaghan Psychotherapy Services BA Life Recovery
Center
Enclosed for Board approval is an Assumption of a Child Protection Agreement between the
Department and Monaghan Psychotherapy Services DBA Life Recovery Center. This
Assumption Agreement was reviewed under the Board's Pass-Around Memorandum dated
January 21, 2014, and approved for placement on the Board's Agenda.
Counseling Services of Longmont will now operate under Monaghan Psychotherapy Services DBA
Life Recovery Center. No other changes have been made. Monaghan Psychotherapy Services DBA
Life Recovery Center will assume all rights and responsibilities under the current agreement
(PY-13-14-CPS-0119)that provides Anger Management/Domestic Violence Evaluation and
Treatment, Mental Health Services, Monitored Sobriety Services, and Substance Abuse
Evaluation and Treatment.
Funding is from the Child Welfare Administration fund. The term is November 1, 2013 through
May 31, 2014.

ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES
Whereas,Counseling Services of Longmont,Inc., (the"Contractor"),and the Board of Weld County
Commissioners, on behalf of the Weld County Department of Human Services (the"Department"),entered into a
Child Protection Agreement for Services(PY-13-14-CPS-0119), dated November 18,2013,incorporated herein
by this reference(the"Agreement"),wherein Contractor will provide Anger Management/Domestic Violence
Evaluation and Treatment;Monitored Sobriety Services; and Substance Abuse Evaluation and Treatment,and;
Whereas,Contractor desires to assign all of its rights and obligations under the Agreement to Monaghan
Psychotherapy Services,Inc.DBA Life Recovery Center; and;
Whereas,Monaghan Psychotherapy Services,Inc.DBA Life Recovery Center intends to benefit the
Department by promising to perform all terms and conditions of the Agreement.
Now,therefore,in consideration of the Department's approval and other mutual covenants and
agreements expressed in the Agreement,Monaghan Psychotherapy Services,Inc.DBA Life Recovery Center
does hereby covenant and agree to assume all obligations,responsibilities, and terms of the Agreement and
hereby becomes the Contractor under the Agreement.
Further,Monaghan Psychotherapy Services,Inc.DBA Life Recovery Center submits to the Department
the following notice of address pursuant to Section 17 of the Agreement:

Further,Monaghan Psychotherapy Services,Inc. DBA Life Recovery Center designates to the
Department the following Contractor representative pursuant to Section 16 of the Agreement:

Owner/Lead Clinician
Further,Monaghan Psychotherapy Services,Inc. DBA Life Recovery Center submits to the Department
herewith,the proof of insurance as required in Section 9 of the Agreement.
