RE: APPROVE ADDENDUM TO AGREEMENT FOR PROFESSIONAL SERVICES AND
AUTHORIZE CHAIR TO SIGN - DUANE DURAN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Addendum to the Agreement for
Professional Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Sheriff's Office, and Duane Duran,
with terms and conditions being as stated in said addendum.
WHEREAS, after review, the Board deems it advisable to approve said addendum, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Addendum to the Agreement for Professional Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Sheriff's Office, and Duane Duran be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said addendum.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of December, A.D., 2014.

Sterling Geesaman has made the following request to increase the jail chaplain from 30 hours to 40 hours per week. It
would increase the cost$14,560. If the amendment is done in 2014 it will not require a supplemental appropriation.
Recently the Sheriff's Office contracted a new Inmate Services Chaplain, Duane Duran. He is performing very well and
making a positive impact. Based on his enthusiasm and the needs of the inmate population,Chaplain Duran is unable to
accomplish everything required within the current weekly contract hours. There is a need to increase the contract hours
from up to 30 hours a week to up to 40 hours a week. The contracted hourly rate for the Chaplain is$28.00 an hour.
The budget impact would be up to an additional$14, 560.00 a year if the Chaplain worked 40 hours every week of the
year.
I am requesting that section 4. Compensation in the contract is modified to state: County agrees to pay
Contractor for services performed as set forth on Exhibit A at the rate of$28.00 per hour/not to exceed 40 hours per
week. Agreement shall not exceed$58,240.00.
Attached is the current contract.

ADDENDUM TO AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN WELD COUNTY AND DUANE DURAN

This Addendum to the Agreement is made and entered into this �5 day of December,
2014 by and between the County of Weld, a body corporate and politic of the State of Colorado,
by and through the Board of County Commissioners of Weld County, Colorado, hereinafter
referred to as"County," with offices located at 1150 'O' Street, Greeley, Colorado 80632, and
Duane E. Duran, hereinafter referred to as "Contractor", whose address is 1130 2"d Street Road,
Eaton, Colorado 80615 .
WHEREAS,the parties entered into an Agreement for Professional Services on
September 29, 2014, (referenced by Weld County Clerk to the Board document number 2014-
2973), hereinafter called the "Agreement"; and
WHEREAS, Section 4 of the Agreement provides for the compensation to be paid,
maximum hours per week to be compensated, and maximum annual payment to be made under
the Agreement, and
WHEREAS, the County and the Contractor desire to amend the Agreement as
specifically described in this Addendum, below.
NOW, THEREFORE, it is agreed by the Parties that the following changes are made to
the aforementioned AGREEMENT as follows:
1. Section 4 of the Agreement shall be replaced in whole with the following language:
"Compensation. County agrees to pay Contractor for services performed as set forth on Exhibit
A at the rate of$28.00 per hour/not to exceed 40 hours per week. Charges shall be based on the
time actually spent performing the services, but shall exclude travel time. Mileage may not be
charged to and from any required job site. Contractor shall not be paid any other expenses unless
set forth in this Agreement. Payment to Contractor will be made only upon presentation of a
proper claim by Contractor, itemizing services performed. Payment for services and all related
expenses under this Agreement shall not exceed $58,240.00."
2. All other terms and conditions set forth in the Agreement shall remain the same.
