RE: APPROVE 2015 ANNUAL PLAN AND BUDGET FOR AREA AGENCY ON AGING
PROGRAMS (TITLE III AND VII) AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with the 2015 Annual Plan and Budget for Area
Agency on Aging Programs(Title III and VII) between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Colorado Department of Human Services,
commencing July 1, 2014, and ending June 30, 2015, with further terms and conditions being as
stated in said plan and budget, and
WHEREAS, after review, the Board deems it advisable to approve said plan and budget,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the 2015 Annual Plan and Budget for Area Agency on Aging Programs
(Title III and VII) between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging, and the Colorado Department of Human Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said plan and budget.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 2nd day of June, A.D., 2014.

RE: Federal and State FY 2015 Budget between the Weld
County Department of Human Services' Area Agency on
Aging and the Colorado Department of Human Services,
Division of Aging and Adult Services
Enclosed for Board approval is the 2015 Federal and State Budget between the Weld County
Department of Human Services' Area Agency on Aging and the Colorado Department of
Human Services, Division of Aging and Adult Services. This Budget was reviewed under
the Board's Pass-Around Memorandum dated May 22, 2014, and approved for placement on
the Board's Agenda.
The FY15 budget supports the following programs for Weld County older adults: the
congregate meal program, legal program, the long term care ombudsman, Hispanic senior
outreach, peer counseling, homemaker and personal care, home delivered meals, health
promotion,dental,vision and hearing services,chore services,caregiver support, grandparents
raising grandchildren services, information and assistance, health promotion, adult day care,
senior center operations, administration expenses, etc.
The budget for FY 2015 is $1,637,991.00 for the period of July 1, 2014 through June 30,2015.
