RE: APPROVE WORK ORDER FORM (FLOOD REPAIRS -WEL-BR27.5/48A) PURSUANT
TO AGREEMENT FOR DESIGN SERVICES AND AUTHORIZE CHAIR TO SIGN -
AYRES ASSOCIATES, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
WEL-BR27.5/48A) Pursuant to Agreement for Design Services between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf
of the Department of Public Works, and Ayres Associates, Inc., commencing January 13, 2014,
with further terms and conditions being as stated in said work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - WEL-BR27.5/48A) Pursuant to
Agreement for Design Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Public
Works, and Ayres Associates, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said work order form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 3rd day of February, A.D., 2014, nunc pro tunc January 13, 2014.

EXHIBIT A - SCOPE OF WORK
Weld County 2013 Flood Recovery
WEL027.5-48.0A County Road 27.5 over Big Thompson River
Structure Description:
This three-span bridge is 153.5 feet long with three nearly equal-length spans. The bridge has
double-tee concrete girders and concrete abutments with flared wingwalls. The piers and
abutments are believed to be founded on piles but this has not been verified. The concrete piers
are continuous walls under the full width of the bridge deck. The 2010 bridge inspection report
gave a Sufficiency Rating of 96.8 and an Item 113 (scour critical) code of 5. which means the
structure was expected to be stable under anticipated scour conditions. and that the estimated
scour line is within the piles (e.g. below the pile cap) of at least one substructure element.

Flood Damages and Ongoing Issues:
Ayres staff visited the crossing site with Richard White of Weld County on January 16, 2014.
The main damage sustained at this crossing was caused by road overtopping over large
stretches of the road both north and south of the bridge. The nature of the damage was erosion
of the embankment side slopes (mostly along the east side of the road but also some along the
west side) along with undercutting of the paved shoulders.
This site is located in an area where the river channel is highly sinuous. The river channel
pattern here is bimodal. in that it is composed of a small low flow channel meandering within a
larger high-flow meandering course. There are numerous historic and pre-historic cutoffs
located on the floodplain upstream and downstream of the bridge. These cutoffs indicate that
the channel in this reach is highly active with regard to meander migration.
Given that the roadway and bridge are located within the neck of a large meander loop and that
the banks of the upstream and downstream limbs of the loop are as close as 300 feet, the
potential for a neck cutoff across the roadway during the next major flood that overtops the
roadway is a distinct possibility. It is especially concerning that the lowest point in the roadway,
which is nearly level with the floodplain, is also coincident with the closest proximity of the
upstream and downstream limbs of the river.

Work Recommended Prior to Spring Runoff(Phase 1):
• Armor the toes of the abutments and wingwalls, along with the lower portions of the
embankment side slopes, with riprap to a distance of 25 feet beyond the wing walls
• Survey the right of way and the roadway within the right of way from 1600ft north to 600ft
south of the bridge
Work Anticipated for Phase 2:
• Regrade and protect the east roadway embankment north (1600 ft) and south of the
bridge (550 ft) with TRM and rock (see sketch below) to prevent more roadway damage
in case of an exceptionally high spring runoff discharge
• Replace the roadway patch
• Protect the bank in the bendway that comes near the roadway on the east side
• Remove or lower the point bar in the vicinity of the bridge

Potential Phase 3 work for Betterments and/or Resiliency:
• Place a guidebank on the right bank upstream of the south abutment
• Build a crossing (or crossings) at the low point in the road north of the bridge (If a larger
crossing looks probable, don't protect the bendway in Phase 2 —just monitor it)
Detailed Scope of Work for Phase 1:
1. Kickoff Meeting. Ayres project manager/engineer and geomorphologist will meet with
County project manager at County Public Works office.
2. Site Assessment. Ayres project manager/engineer and geomorphologist meet at the site
with County project manager to observe current field conditions. assess geomorphology
and discuss what emergency repairs were done.
3. Survey. Conduct boundary survey of the right of way limits and topographic survey of the
road pavement and embankment within the right of way from 1600ft north to 600ft south
of the bridge with greater detail around the bridge abutments.
4. Prepare brief environmental documentation for permitting the work at this site.
5. Progress Meeting. Conduct one progress meeting with County project manager at
County Public Works office.
6. Design. Develop design of Phase 1 riprap work.
7. Plans and Deliverables. Prepare abbreviated Phase 1 construction documents (following
CDOT "writeup plans" format). Estimate quantities and prepare engineer's estimate of
probable cost for Phase 1 activities. Also prepare a ROW Plan for CDOT's review to
cover anticipated work through Phase 2.
