WHEREAS, the Weld County General Hospital Board of Trustees, and
Four-Phase Systems, Inc. have entered into an agreement whereby Four-
Phase Systems, Inc. guarantees to lease certain computer equipment to the
Weld County General Hospital pursuant to said lease agreement marked
Exhibit A, and
WHEREAS, the Board of County Commissioners, Weld County, Colorado,
has determined that said agreement providing for the lease of certain computer
equipment to the Weld County General Hospital would be in the best interests
of the County and the Weld County General Hospital in accordance with the
terms and conditions thereof.
NOW, THEREFORE, BE IT RESOLVED, that the lease agreement with
the Weld County General Hospital Board of Trustees and Four-Phase Systems,
Inc. , copy of which is attached hereto and made a part hereof, be approved.
BE IT FURTHER RESOLVED, that the Chairman of the Board be, and
he hereby is authorized to execute the approval of said agreement and to
make the same effective forthwith.
The above and foregoing resolution was, on motion duly made and seconded,
adopted by the following vote on the 2nd day of June, A.D. , 1976.

This LEASE AGREEMENT is made by Four-Phase Systems, Inc. (hereinafter referred to as Four-Phase Systems),
and
(hereinafter referred to as Lessee).
1. SUBJECT MATTER OF LEASE - In accordance with the terms and conditions of this Lease Agreement,
Four-Phase Systems hereby agrees to lease to Lessee and Lessee hereby agrees to lease from Four-Phase Systems the
