I have done a review of the packet and have a few items that need corrected/finalized. Unfortunately, Tammy attached the wrong Report of Changes form and requested the wrong fee, so I'm providing the correct form to be completed. I think you'll see this one has a section that is more specific to a Manager Change. Page 2 provides instructions on completing the form and I will need a new check for the local fee is $75, payable to: Weld County Clerk to the Board (I already have one $75 check that will go to the "Colorado Department of Revenue". I will return the $100 check). Once you have completed the form, please be sure it is signed and dated. There are two sets of fingerprint cards and we only require one, so one of those will be returned. Lastly, on the Individual History Record - question #4 requires the Manager's personal mailing address, if different from #5, so that can be changed to N/A, rather that listing the business address. #13.r asks for the current Driver's License No. and State. Also, I need to have Kory sign and date before I can process the request. Thanks for addressing these items and please call if you have any questions.
