OPTIONS: Change in amount of services/goods in conjunction with renewal for additional
term.
REQUIRED PROVISIONS. In accordance with the Original Contract routing number 11
IHA 18055 between the State of Colorado, Department of Human Services, Employment and
Benefits Division and Weld County Area Agency on Aging, the State hereby exercises its option
for an additional term beginning July 1, 2014 and ending on June 30, 2015, AND a Decrease in the
amount of goods/services.
The amount of the current Fiscal Year 2015 (SFY15) contract value shall be $52,103.74. (The Home
Care Allowance program for SFY 2015 shall be $51,598.15; the Special Populations Home Care
Allowance program for SFY 2015 shall be $505.59 as consideration for services/goods ordered under
the contract for SFY 15. The total contract value including all previous amendments, options letters,
etc. is $238,718.68; (State Fiscal Year 11 $38,781.01; State Fiscal Year 12 $43,439.48; State Fiscal
Year 13 $50,048.49 and State Fiscal Year 14 $54,345.96. Page 1, the Contract price not to exceed is
hereby modified accordingly.
, Effective Date. The effective date of this Option Letter is upon approval of the State
Controller or July 1, 2014, whichever is later.

CRS§24-30-202 requires the State Controller to approve all State Contracts. This Contract is not valid
until signed and dated below by the State Controller or delegate. Contractor is not authorized to begin
performance until such time. If Contractor begins performing prior thereto, the State of Colorado is not
obligated to pay Contractor for such performance or for any goods and/or services provided hereunder.
