RE: APPROVE ADDENDUM #19 TO AGREEMENT FOR SERVICES AND AUTHORIZE
CHAIR TO SIGN -COLORADO TECHNICAL SERVICES, INC. (CTSI)
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Addendum #19 to the Agreement for
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, and Colorado Technical Services, Inc. (CTSI), commencing
upon January 1, 2015, and ending on December 31, 2015, with further terms and conditions being
as stated in said addendum, and
WHEREAS, after review, the Board deems it advisable to approve said addendum, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Addendum #19 to the Agreement for Services between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
and Colorado Technical Services, Inc. (CTSI), be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said addendum.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of October, A.D., 2014.

RE: Agreement for CTSI Services-2015 WELD COUNTY
COMMISSIONERS
Dear Commissioner Rademacher:
We are pleased to continue to provide claims administration services for Weld County's
workers' compensation claims. Enclosed are the addenda to the Agreement for CTSI Services
with Weld County for services from January 1 through December 31, 2015. The overall fee was
increased by 2.5%which reflects the operational cost increases incurred by CTSI for claims
administration services as well as the regional rate of inflation for Denver, Boulder and Greeley
based on the Consumer Price Index and other sources.
Please sign and return one set of originals to me and keep one set for Weld County.

Agreement for CTSI Services
Addendum 19
Service Fees-2015
Weld County shall pay CTSI the fees as listed in this Addendum-Service Fees to this agreement
for workers' compensation claims administration and other services rendered under this
Agreement for the period January 1, 2015 through December 31, 2015.
Workers' Compensation Claims Administration
Per Claim Fee
An annual workers' compensation claims administration fee based on an estimated annual claim
volume shown below.

If the claim volume exceeds this estimate,the additional claims will be billed at the

EDP Programming Fee
A programming fee of$118 per hour plus expenses for claim reports requiring special
programming.
Claim Handling Time Period
Fee is for handling the claim to conclusion so long as a contractual relationship exists with CTSI
for third-party claim administration services. A deposit equal to an average month of claim
expense shall be placed with CTSI for payment of claims on behalf of Weld County.
Claims Run-Off
If Weld County terminates its contract with CTSI and decides to have CTSI administer the run-
off of claims,the open claims will be administered at a per claim rate of 90%of our current per
claim charge at the time of termination.
