Subject:Acceptance of Weld County Collaborative Management Program MOO for SFY 2014-2015
Dear Director Griego:
The purpose of this correspondence is to communicate to you information about participation in the Collaborative
Management Program (CMP), CRS 24-1.9-101, also known as HB 04-1451.
The Weld County CMP MOO for SFY 2014-2015 is accepted. Weld County has elected to participate in surplus
distribution. Weld County has not requested any waivers.

As specified in the MOO, the Weld County CMP plans to serve approximately 2,896 children, youth, and their
families at risk of becoming involved in the child welfare or juvenile justice systems.
Appreciation is extended for the continuing commitment of the Weld County CMP to coordinate and integrate
services for children, youth and families in order to better meet their complex needs.
