RE: APPROVE WORK ORDER FORM (FLOOD REPAIRS - EM-BR23/14A) PURSUANT TO
AGREEMENT FOR CONSTRUCTION ADMINISTRATION SERVICES FOR
PERMANENT REPAIRS AND AUTHORIZE CHAIR TO SIGN - BOHANNAN HUSTON,
INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
EM-BR23/14A) Pursuant to Agreement for Construction Administration Services for Permanent
Repairs between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Works, and Bohannan
Huston, Inc., commencing August 4, 2014, with a completion date of September 18, 2014, and
with further terms and conditions being as stated in said work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - EM-BR23/14A) Pursuant to
Agreement for Construction Administration Services for Permanent Repairs between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Public Works, and Bohannan Huston, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said work order form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of July, A.D., 2014.

Services Work Orders
Weld County, Public Works is proposing using Bohannan Huston Inc. (BHI) for daily inspection
associated with the Dry Creek Flood Recovery construction project. BHI is already under

is scheduled to take place from August to mid September of this year, and will return 9 sites to
pre-flood conditions. Weld County will be eligible for reimbursement from FEMA for the cost of
using Bohannan Huston Consultants for daily inspection of this project.
Recommendation:
Authorize execution of the Construction Inspection Work Orders with BHI in the amount of
$33,050.00 so that Public Works staff will have time to work on other projects.
The Bohannan Huston Inc. Inspection Proposal and work orders for each site are attached.

Dear Richard -
Based on meetings with Weld County in association with the project noted above,
Bohannan Huston, Inc. (BHI) appreciates the opportunity to submit this response to the
Construction Services Work Orders request from the County.
Upon a thorough review of the scope and our subsequent scoping meeting to discuss our
understanding of the major project elements; we have developed the attached,full scope of
services,fees, and schedule. The following Exhibits are included with this letter:
A. Detailed Scope of Services
BHI has prepared a detailed Engineer's Scope of Services(Exhibit A). These items are
based on our current understanding of the project,and we welcome the opportunity
to discuss the scope herein if it does not meet your expectations.
B. Fee Breakdown
BHI has prepared a detailed Hourly Breakdown and Cost Estimate (Exhibit B)
detailing hours for each team member including reimbursable expenses (Direct
Costs).
C. Schedule
It is our intent to adhere to the Weld County and selected contractor's schedule to
the best of our ability, with the construction tasks herein to be completed in the
approximate timeframe of Mid July through late August 2014.

Engineer's Scope of Services-Construction
Weld County Flood Recovery—Big and Little Dry Creek Corridors
BASIC SERVICES
The Engineer's(Bohannan Huston, Inc.or BHI) Basic Services shall consist of the items described below. We
understand that it is Weld County's intent to construct per plans for the bridge sites to meet the original,
pre-flood conditions. The following scope of services Ls anticipated for each bridge site as detailed in Exhibit
B.
Description of Work
This work consists of performing construction observation and related documentation for the bridge
structure bank and channel repair of nine bridge sites on Big and Little Dry Creeks in Weld County,Colorado.
The project will repair damage caused by the flooding in the fall of 2013 and consists of adding riprap, re-
grading,and other necessary repairs to the areas around nine bridges.
The scope of work will be for a full time construction inspector who will be responsible for the day to day
activities assigned by the County Project Manager;this includes inspecting and documenting all construction
activities, and preparation of daily diaries documenting construction activities and relevant observations in
accordance with CDOT and Weld County standards.
Project Standards
Inspection shall be in accordance with the Colorado Department of Transportation(CDOT)Construction
Manual and the applicable Standard Specifications for Road and Bridge Construction,Project Standard
and Special provisions,project plans,CDOT M&S Standards,and applicable Weld County standards.
General Requirements
Initial Project Meeting
The Consultant and County project personnel shall meet to coordinate and schedule the required
work prior to active construction. The Consultant shall complete all work in accordance with the
approved schedule or as approved by the Engineer.
Work Duration
The time period for the work described in this scope of work will last for approximately 45 calendar
days. Duration of work covered by this scope of work is approximately Mid July 2014 through late
August 2014.

Labor,Materials,Vehicles and Equipment
The Consultant shall furnish all personnel, materials, equipment and transportation required to
perform the work. Vehicle mileage will be actual miles driven on project-related tasks and travel to
and from the consultants' office. Consultant personnel shall have appropriate vehicles, cellular
phones,computers and miscellaneous equipment and supplies(printers,calculators,manuals,office
supplies, safety equipment, etc.) required to perform the work. No Field Office will be provided
with this work.
Personnel qualifications, staffing level, and number and types of vehicles shall be subject to the
approval of the County Project Manager. The Consultant shall assign personnel for the duration of
the Construction Contract unless otherwise approved by the County Project Manager.
The individual assigned to the site will be familiar with CDOT and Weld County specifications,
manuals, forms and documentation requirements and shall be under the direction of, and shall be
reviewed by the County Project Manager. A consultant supervisor shall be available to review work,
resolve problems, and make decisions in a timely manner as requested by the County Project
Manager,and will be experienced and competent in road and bridge construction,and inspections.
Routine Billing and Reporting
The Consultant shall provide the following on a regular basis:
o Monthly billing formats,suitable to the Project Engineer,for all contract activities
performed by the Inspection Consultant.
o Periodic reports and billings required by CDOT Procedural Directive 400.2.
o Weekly time cards for consultant personnel.
o Supporting documentation for all direct costs.
o Weekly approval of mileage for vehicle.
Status of the Project
The consultant shall monitor the status of the project, and advise the Project Engineer of any
potential need for supplementing their contract.
Construction Inspector Requirements
Inspection work shall conform to the CDOT Construction Manual and the Inspector Checklist. Each
of the Consultant's field inspectors will maintain a daily diary for each day the Consultant performs
work on the project. They shall use CDOT's Form 103 or automated 103a - Project Diary, or other
form approved by the County Project Manager. The contents of the diary shall be brief and accurate
statements of progress and conditions encountered during the prosecution of the work. Editorial
comments are not to be incorporated in the diaries or on any written correspondence applicable to
the project. A copy of the daily diary shall become a part of the permanent project record.
