HomeMy WebLinkAbout20143024.tiff RESOLUTION RE: APPROVE COMMUNITY SERVICES BLOCK GRANT(CSBG)CONTRACT AMENDMENT #2 AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with a Community Services Block Grant(CSBG) Contract Amendment#2 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,and the Colorado Department of Local Affairs, commencing upon full execution of signatures, and ending September 30, 2015, with further terms and conditions being as stated in said agreement, and WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Community Services Block Grant (CSBG) Contract Amendment #2 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,and the Colorado Department of Local Affairs be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said amendment. Griego, Director, Hum Servi es ep t RE: Amendment #2 to the 2012-20 5 Community Services Block Grant between the Weld County Department of Human Services and the Department of Local Affairs Enclosed for Board Approval is Amendment#2 to the 2012/2015 Community Services Block Grant (CSBG)between the Department and the Department of Local Affairs. This CSBG Amendment was reviewed at the Board's Work Session of September 18, 2014. The Board previously approved the addition of employment and educational activities to the Community Services Block Grant through its plan. The purpose of this contractual amendment is to finalize that process. This contract is specifically for the Community Services Block Grant fiscal year 2014-2015. The Department's allocation for this fiscal year is $286,516. The term of this Amendment is effective the date of execution through September 30, 2015. If you have questions, please give me a call at extension 6510. 2014-3024 CSBG #44 —Weld County GRANT AMENDMENT Amendment#: 2 Encumbrance#: L12CSBG44 Original Contract CMS/CLIN #: 47096, 53699, 59237 Amendment CMS#: 66707 1) PARTIES This Amendment to the above-referenced original grant agreement(hereinafter called the "Grant") is entered into by and between WELD COUNTY (hereinafter called"Grantee"), and the STATE OF COLORADO (hereinafter called the "State") acting by and through the Department of Local Affairs, (hereinafter called "DOLA"). 2) EFFECTIVE DATE AND ENFORCEABILITY This Amendment shall not be effective or enforceable until it is approved and signed by the Colorado State Controller or designee (hereinafter called the"Effective Date"). The State shall not be liable to pay or reimburse Grantee for any performance hereunder including, but not limited to, costs or expenses incurred, or be bound by any provision hereof prior to the Effective Date. 3) FACTUAL RECITALS The Parties entered into the Grant to provide funding for services and/or assistance to low income families and individuals. The Grant covers a three-year period: 03/01/12 through 09/30/15. This amendment adds a new subgrantee and eligible expenses, modifies the eligible expenses for the Grantee, and provides an allocation of Year 3 funding to Grantee. 4) CONSIDERATION-COLORADO SPECIAL PROVISIONS The Parties acknowledge that the mutual promises and covenants contained herein and other good and valuable consideration are sufficient and adequate to support this Amendment. The Parties agree to replacing the Colorado Special Provisions with the most recent version (if such have been updated since the Contract and any modification thereto were effective) as part consideration for this Amendment. 5) LIMITS OF EFFECT This Amendment is incorporated by reference into the Grant, and the Grant and all prior amendments thereto, if any, remain in full force and effect except as specifically modified herein. 6) MODIFICATIONS The Grant and all prior amendments thereto, if any, are modified as follows: a. Exhibit B, 1. Project Description is modified by deleting the section in its entirety: "1. PROJECT DESCRIPTION Grantee shall provide assistance or services from the list of Federal Objective(s) for the CSBG Program Employment, Emergency Services and Linkages. 1.1 Grantee. Grantee shall provide services and activities through the Workforce Center that address the Federal Objective Employment by providing job development services to assist low-income participants to remove or reduce barriers to initial or continuous employment; and Grantee shall provide services and activities meeting the Federal Objective Emergency Services by providing one-time emergency assistance to stabilize households; and Grantee shall provide services and activities meeting the Federal Objective Linkages by administering a senior services program, which provides senior nutrition sites and training to volunteers. 1.2 Eligible expenses for Employment are: salary and benefits of the case manager and teachers, eyeglasses and repairs, physicals, uniforms or special clothing or tools needed to start a job, immunizations, driver's license or state issued identification, background checks including fingerprints, gas vouchers,bus passes, educational supplies for job training, ESL/GED books and testing fees, interview language translation services, training and job search supplies(paper, pens, notebooks, folders and markers). 1.3 Eligible expenses for Emergency Services are: medical care and supplies, eye exams, lenses, frames, frame repairs,dental care, prescriptions,food, rent/mortgage assistance, motel vouchers, vouchers to homeless shelters, gas vouchers, utility assistance(water, sewer, electricity,gas, wood, pellets, propane), primary phone, utility repair or replacement of furnace and hot water heaters up to $500 with prior DOLA approval, client moving expense up to $500 with prior DOLA approval, clothing, furniture/appliance expenses up to$500 with prior DOLA approval, identification documents, bus passes, child care assistance and related transportation costs,vehicle repairs up to$500 with prior DOLA approval, client car insurance and vehicle registration and emissions testing. 1.4 Eligible expenses for Linkages are: volunteer driver supplemental insurance premiums, a portion of salary and benefits for the director and technicians working with CSBG eligible clients. 1.5 Service Area.The performance of the Services described within this Grant shall be located in Weld County." and inserting the following in lieu thereof: `1. This subgrantee shall receive a subaward from Grantee to provide services and activities that address the Education Federal Objective by providing youth enrichment programs, mentoring,tutoring, and providing access to nutritional foods and recreation. Eligible expenses are: Salary and benefits of program coordinators. 1.3 Service Area. The performance of the Services described within this Grant shall be located in Weld County." c. Exhibit C, Project Budget 2. Year 2 is modified by deleting the current subsection in its entirety: "2. Year 2 Budget Category Amount Direct Operating and Personnel Costs $124,348 Indirect Operating and Personnel Costs $11,750 Subgrantee Costs TOTAL $136,098" and inserting the following in lieu thereof: "2. Year 3 Budget Category Amount Direct Operating and Personnel Costs $281,016 Indirect Operating and Personnel Costs $5,500 Subgrantee Costs TOTAL $286,516" 8) START DATE This Amendment shall take effect on the later of its Effective Date or August 31, 2014. 9) ORDER OF PRECEDENCE Except for the Special Provisions, in the event of any conflict, inconsistency, variance, or contradiction between the provisions of this Amendment and any of the provisions of the Contract, the provisions of this Amendment shall in all respects supersede, govern, and control. The most recent version of the Special Provisions incorporated into the Contract or any amendment shall always control other provisions in the Contract or any amendments. 10) AVAILABLE FUNDS Financial obligations of the state payable after the current fiscal year are contingent upon funds for that purpose being appropriated, budgeted, or otherwise made available.
