Please review and indicate if you would like a work session prior to placing this item on the Board's
agenda.
Request Board Approval for the Departments'Child Protection Agreement Extension/Renewal
with Weld County School District RE-8.The parties agree to extend the term and rate structure of their
original agreement(2014-3047),under the standard form for renewals.The standard form was approved
as new business on March 9,2015.The renewal term is for June 1,2015 to May 31,2016.
Under this renewal,the parties agree to changes in scope of services.These changes are as follows:
EXHIBIT A-SCOPE OF SERVICES,Paragraph 5, Item F,is hereby amended as follows:
Human Services,as the Lessor,will provide"Office Space",located in the Weld County Department of
Human Services Building at 315 North I la Avenue,Greeley,Colorado,hereinafter n,fiered to as'The
Premises,"until such time that Human Services no longer offices at 315 North I I"Avenue,as follows:
1. Human Services,as the I essor,will provide one shared work space to Contractor,as Lessee,which
will be located at the 315 North I 1"Avenue and will provide one work space to be located at the
Premises,until such time that Human Services no longer offices at 315 North 1 l a Avenue.
EXHIBIT A-SCOPE OE SERVICES, Paragraph 6, is hereby added to as follows:
Contractor agrees that the assigned or selected FTE will attend scheduled Family Team Meetings,Team
Decision Making Meetings,and Group Supervisions as assigned by the Prevention Programs Supervisor or
other Human Services management.
I do not recommend a Work Session. I recommend approval of this Renewal.

AGREEMENT EXTENSION/RENEWAL BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND WELD COUNTY SCHOOL DISTRICT RE-B(CORE)

This Agreement Extension/Renewal("Renewal"),made and entered into G.27 day of —1-0414(,2015,by and between
the Board of Weld County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter referred to as
the"Department",and Weld County School District RE-8, hereinafter referred to as the"Contractor".
WHEREAS the parties entered into an agreement(the"Original Agreement") identified by the Weld County Clerk to the
Board of County Commissioners as Document Number 2014-2815,approved on 09/15/2014.
WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the
Original Agreement,which is incorporated by reference herein,as well as the terms provided herein.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on May 31,2015.
• The parties agree to extend the Original Agreement for an additional one(1)year period,which will begin June 1,2015, and
will end on May 31,2016.
• The Renewal,together with the Original Agreement,constitutes the entire understanding between the parties. The
following change is hereby made to the Contract Documents:

Request Board Approval for the Departments' Child Protection Agreement
Extension/Renewal with Rick L. May, Psy.D,P.C.,DBA Treatment& Evaluation
Services. The parties agree to extend the term of their original agreement(2014-3047), under
the standard form for renewals. The standard form was approved as new business on March 9,
2015. The renewal term is for June I,2015 to May 31,2016.
Under this renewal,the parties agree to changes in scope of services and the rate structure.
These changes are as follows:
EXHIBIT B-SCOPE OF SERVICES: Psychosexual Evaluations for Juveniles and Adults
— Evaluations are conducted over one (I) four-five hour appointment. Contractor will
review documentation, contact collateral sources (e.g., GALs, therapists, etc.), Written
report will be provided to the Department and Contractor will hold a feedback session
after the report is completed to review the results of the evaluation.
Psychological Evaluations (Sexual Focus, Children 13 and younger) — Evaluations are
conducted over 4-5 hours over 2 days due to children's attention span. Contractor will
review documentation, contact collateral sources (e.g., GALS, therapists, etc.).
Written report will be provided to the Department and Contractor will hold a feedback
session after the report is completed to review the results of the evaluation. Children
cannot complete many of the tests associated with a psychosexual evaluation, therefore a
psychological evaluation with a sexual focus is conducted and other testing is utilized such
as Rorschach. Thematic Apperception Test, etc.
Psychological Evaluations (All Ages)—Conducted over 4-5 hours of testing. Contractor
will review documentation, contact collateral sources (e.g.. GALs, therapists, etc.).
Contractor will conduct testing appropriate for the referral and purpose of the
evaluation, as well as the presenting issue(s). A written report will be provided to the
Department and Contractor will hold a feedback session after the report is completed to
review the results of the evaluation.
Juvenile Offense Specific Treatment—Juvenile offense specific treatment is provided per
50MB standards with a length of stay determined by the individual needs of the referred
juvenile. Both individual and group treatment is generally provided with a minimum of

one (I)group per week and one(1)to two (2) individual sessions per month.
Boundaries Treatment(Adults and Juveniles)—Treatment is offered either individually or in
a group setting. Topics include decision-making,communication skills, family dynamics,
emotion management, values clarification,empathy, boundaries, healthy sexuality, etc.
The expected length of treatment is 8-10 sessions.
Contractor employees a Spanish-speaking SOMB provider and can conduct evaluations
and treatment in Spanish. Treatment in Spanish is only available at the Aurora location.
EXHIBIT C-PAYMENT SCHEDULE, PARAGRAPH 2-FEES FOR SERVICE:
• $850.00/Episode (Psychosexual Evaluations for Juveniles and Adults)
• $900.00/Episode (Psychological Evaluations-Sexual Focus, Children 13 and
younger)
• $1,200.00/Episode (Psychological Evaluations, All Ages)
• $35.00/Hour (Offense Specific Group-1.5 Hours/Group, Boundaries Group-I
Hour/Group)
• $80.00/Hour(Individual Session. Court Facilitation/Court Staffing/Family Team
Meeting/Team Decision Making Meeting)
