REVIEW (USR) QUESTIONNAIRE
The following questions are to be answered and submitted as part of the USR application. If a question does not pertain to your
use, please respond with "not applicable", with an explanation as to why the question is not applicable.
1 . Explain, in detail, the proposed use of tibe property. - The property is proposed for the owner's home/farm and trucking
company
2. Explain how this proposal is consistent with the intent of the Weld County Code, Chapter 22 (Comprehensive Plan). -
Currently the neighboring properties to the North, East and West have various businesses from dog kennels and other
Ag businesses to oil field/trucking businesses. This business is off the main road and out of view from all but the
adjacent neighbors. Vehicle traffic will be minimal 15 max trucks 1-trip/day.
3. Explain how this proposal is consistent with the intent of the Weld County Code, Chapter 23 (Zoning) and the zone district in
which it is located. — The existing zoning is Agricultural which allows farm equipment storage and maintenance shops.
The proposed uses would not be significantly different than a farm use, with minimal disruption to the surrounding area.
4. What type of uses surround the site? Explain how the proposed use is consistent and compatible with surrounding land uses. -
East of the property is a large kennel business with training tracks and storage, west and north are various small
businesses, including more kennels and rabbit - worm farm operation and trucking businesses. This use is similar to
other trucking operations within a mile of this property.
5. Describe, in detail, the following:
a. How many people will use this site? — Maximum of 2 employees and the owner's family.
b. How many employees are proposed to be employed at this site? — 17 including drivers

c. What are the hours of operation? — 6:00 am — 10:00pm, 7 days a week
d. What type and how many structures will be erected (built) on this site? — The owner is requesting a home, 2 car
garage, a barn for animals and a shop to do minimal maintenance to the trucks.
e. What type and how many animals, if any, will be on this site? 6 horses -- 4 cows
f. What kind (type, size, weight) of vehicles will access this site and how often? — 15 semi trucks and trailers, 80,000
lbs gross weight, 1-round-trip/day
g. Who will provide fire protection to the site? — Ft. Lupton Fire District
h. What is the water source on the property? (Both domestic and irrigation). — Private Well (Permit Pending)
I. What is the sewage disposal system on the property? (Existing and proposed). — Private septic (Permit Pending)
j. If storage or warehousing is proposed, what type of items will be stored? — Oil field and related equipment
6. Explain the proposed landscaping for the site. The landscaping shall be separately submitted as a landscape plan
map as part of the application submittal. — Since none of this site abuts a developed R.O. W. only the existing trees
lining the entrance will be incIdded.
7. Explain any proposed reclamation procedures when termination of the Use by Special Review activity occurs. — N/A
8. Explain how the storm water drainage will be handled on the site. — No significant change in historical flows will
be created, a water quality pond will be defined in an existing low-lying area where water naturally flows. All
drive and parking surfaces will be gravel or recycled asphalt to allow water absorption.
9. Explain how long it will take to construct this site and when construction and landscaping is scheduled to
begin. - From approvals PHASE 1 - immediately, PHASE 2 - 60 months completed shop, PHASE 3 - 84 months
House, stable and garage.
10. Explain where storage and/or stockpile of wastes will occur on this site. — N/A

USR -- Checklist Narratives
1 Water for Site Use

2 Adequate Sewage Disposal
a . Septic design/engineering and construction of new septic will be completed at time of
permit for home and shop. All necessary compliance with building permitting and fire
compliance to be completed prior to certificate of occupancy for new construction .
b. At time of shop construction an exception to use for commercial restroom, no other
water/sewage use.
3 Drainage
a . Onsite water quality to be developed in existing low-lying area to contain surface runoff.
Area to be approximately as shown on site plan defined by newly graded area.
Historical flows do not significantly impact surrounding land areas, as slopes are minimal
and water contained historically onsite. New surface areas to be pervious so that
historical water runoff will not be increased significantly, proposed water quality pond will
contain additional flows from building development and historical surface water. Soils to
be sandy and minimum clays so percolation will allow water to flow into lower aquifers.
4 Traffic Narrative
a . Existing access approved in RE-4704, continues to serve oil field truck traffic and owner's
trucks. Turning radius as shown by AASHTO interstate semi-trailer of 68. 5' in length
requires an outside radius of 45' . The current site conditions allow for an inside radius of
45' more than adequate for a safe access to the current road from the existing 30' drive.

b. The neighbor's secondary access is just east of the driveway, it appears to be used
regularly, but is not the only access to this site.

USR - Checklist Narratives
1 Water for Site Use
a. Final well permit application has been submitted with required data - waiting for
approval - See attached
b. At time of shop construction an exception to use for commercial restroom, no other
water/sewage use. Primary use of water on site will be domestic, as owner and
family will require the majority of water use. The exemption for commercial use
will only be for 1 -full time employee and owner on site, supplemental use for
drivers, less than 2hrs/day. Because the permitting process does not allow for
both domestic and commercial permitting, the higher use permit was requested —
then at time of shop construction an exemption to the existing well permit/use
will be requested. The minimal water requirement for 1 -additional full-time
employee should not impact the residential allowance significantly and we'll have
approval for the higher domestic water use impact.
2 Adequate Sewage Disposal
a. Septic design/engineering and construction of new septic will be completed at time of
permit for home and shop. All necessary compliance with building permitting and fire
compliance to be completed prior to certificate of occupancy for new construction .
b. At time of shop construction an exception to use for commercial restroom , no other
water/sewage use.
c. Prior to building construction and permanent sanitary facilities, I portable toilet
will be set on site for driver use. Driver on site use is less than 1 hr per driver per
day. This will be leased from a licensed Colorado company and maintained per
Colorado Law.
3 Use of Shop
a. The truck maintenance will be limited to changing of fluids, tires, belts, and other
similar replacement of maintenance parts. No engine replacement or
overhauling.
b. No floor drain in shop - any spillage will be contained with appropriate sweeping
agent, designed to absorb spills for disposal.
c. Used fluids, motor oil will be contained in approved containment barrels for
recycling. Regular pickup scheduled as required for removal of excess waste
fluids - no volumes in excess of 250ga1s TTL to remain on site.
