Request Board Approval for the Departments' Child Protection Agreement Extension/Renewal with Shiloh House(Core/Non-Core).The 2014-15 Shiloh House Core Services agreement(2014-3046) was renewed for 2015-16, under the standard form for renewals. The standard form was approved as new business on March 9, 2015. The renewal term is for June 1, 2015 to May 31,2016. The renewal includes all services approved in 2014-15 as well as additional/new services in the areas of Life Skills and Home Based Intensive Services. Life Skills services under this agreement have been further defined to include Therapeutic Parenting time available at four(4) intensity levels—Intensive,High, Moderate,and Low. Home Based Intensive Services are a new addition to the agreement and are being provided in the form of High Fidelity Wraparound Services. All services,with the exception of Case Management, will be purchased through Core Services. Case Management will be purchased through non-Core funding as it is not reimbursable under Core Services. PAYMENT SCHEDULE-Fees for Services: Life Skills-Therapeutic Parenting Time: $46.00/Hour(Bachelor's Level) $75.00/Hour(Master's Level- In Office) $95.00/Hour(Master's Level - En Home) Home Based Intensive Services-High Fidelity Wraparound Services: $46.00/Hour(In Home/Non-Master's Level) $75.00/Hour(In Office/Master's Level) $95.00/Hour(In-Home/Master's Level) $45.00/Hour(Case Management-Non-Core) All other terms and conditions of the Original Agreement remain unchanged. 2015-2907 ss-Around Memorandum; August 12,2015 Page 1 l do not recommend a Work Session. Approve Request f3OCC A coda Work Session Sean Conway Steve Moreno Barbara Kirkmeyer Mike Freeman Julie Cozad Pass-Around Memorandum;August 12, 2015 Page 2 AGREEMENT EXTENSION/RENEWAL BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND SHILOH HOUSE (CORE/NON-CORE)�/ This Agreement Extension/Renewal("Renewal"), made and entered into - day of ,2015.by and between the Board of Weld County Commissioners,on behalf of the Weld County Department of Human Servi s,hereinafter referred to as the"Department",and Shiloh House,hereinafter referred to as the"Contractor". WHEREAS the parties entered into an agreement(the"Original Agreement")identified by the Weld County Clerk to the Board of County Commissioners as Document Number 2014-3046,approved on 10/06/2014. WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the Original Agreement,which is incorporated by reference herein,as well as the terms provided herein. NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows: • The Original Agreement will end on May 31,2015. • The parties agree to extend the Original Agreement for an additional one(1)year period,which will begin June 1,2015,and will end on May 31,2016. • The Renewal,together with the Original Agreement,constitutes the entire understanding between the parties. The following change is hereby made to the Contract Documents: 1. EXHIBIT C-SCOPE OF SERVICE,Paragraph 3,shall be amended to include: Life Skills: Contractor shall provide services at four levels;Intensive(10-15 Hours/Week), High(7-9 Hours/Weeks), Moderate(4-6 Hours/Week), or Low(3 Hours/Week). Life Skills—Therapeutic Parenting Time: Contractor will provide Therapeutic Parenting Time to families,as referred by the Department. Services provided by a Master's Level clinician may be provided in-office or in-home. Services provided by a Bachelor's Level clinician may only be provided in-office. a. The goal of services is to: • Provide focused therapeutic time for the parent and child within the supervision and therapeutic intervention,training,and skill building of the parenting therapist. • Provide collaboration between the parent and the parenting therapist,following initial assessment and observation, to identify specific skill building areas,as well as development of goals and tasks for each of the parenting times. b. Services are strength-based and trauma-informed. c. Contractor will ensure Bachelor's and Master's Level clinicians shall have training in the following areas: • Trauma Informed Care • Health and Safety • Family-centered Models of Care • Strength-based Treatment Approaches • Cultural Diversity • Mandatory Reporting AGREEMENT EXTENSION/RENEWAL BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND SHILOH HOUSE(CORE/NON-CORE) 2. EXHIBIT C-SCOPE OF SERVICE, Paragraph 3,shall be amended to include: Home Based Intensive Services—High Fidelity Wraparound Services: Contractor will provide High Fidelity Wraparound Services,as referred by the Department. a. Services may be include but are not limited to the following: • Availability and access to effective,community-based services and supports for children and their families that address their emotional,social,educational,and physical needs,including traditional and nontraditional services as well as natural and informal supports. • Individual services in accordance with the needs of each family member,guided by a strengths-based,wraparound service planning a process and an individualized service plan developed with the team with the family prioritizing the needs. • Services and supports that are evidence-informed and established practices. The intervention will be supported by practice-based evidence to ensure effectiveness of services and improve outcomes for youth and their families. • Services and supports will be delivered within the least restrictive, most normative environment that is clinically appropriate. • Ensure referred families and youth are involved in all aspects of the planning and delivery of their own services. • Developmentally appropriate mental health services and supports that promote social- emotional outcomes for youth and their families in their home and community settings. • Developmentally appropriate services and supports to facilitate the transition of youth to the adult services system as needed. • Protection of the rights of youth and families and promotion of effective advocacy efforts. • Services and supports without regard to race, religion,national origin,gender,gender expression,sexual orientation,physical disability,social-economic status,geography, language,immigration status or other characteristics,ensuring services are sensitive and responsive to these differences. b. Referred families shall: • Gain knowledge,confidence and motivation that are important in the process of achieving future goals. • Improve relationships. • Develop and maintain supportive relationships outside the home. • Learn to manage crises. • Decrease the frequency, intensity and duration of crises. c. Contractor will ensure staff have the following qualifications: • Coaches: i. A Bachelor's degree in a Human Services related field. ii. Two years of experience in a Human Services related field. iii. HFW Coach credentialing. iv. A demonstrated understanding of,and have experience with,different systems including schools,mental health,child welfare,juvenile justice, health and safety. v. Experience working with youth having complex emotional needs. vi. Knowledge of community resources. One year of experience providing services to youth and families. viii. HEW facilitator credentialing ix. Experience working with youth having complex emotional needs. xi.
