The Colorado Division of Water Resources(DWR)records indicate that you are the owner/user of the following well(s)("Well(s)")which

The well(s)listed above are included in the scope of the Rules,which became effective December 31,2013 and require
compliance with the Rules no later than December 31, 2015. A copy of the Measurement Rules can be downloaded at:

Please note that the above table only includes wells you own/use that are currently in a court decreed plan of augmentation.
Please contact our office if you believe a well you own/use is not listed above but is in an augmentation plan.
Following is a determination of the current status of the wells listed above: 1)If the above table indicates a Compliance of
GOOD, you are currently in compliance with the Rules and are not required to have a new test until the indicated Expiration
Date; 2)If the above table indicates Inactive, you are currently in compliance with the Rules and are not required to take any
further action until such time as you wish to activate/use your well; or 3)If the Comaltance_Date is 12/3112015. you have_
either not submitted any information to our office or the information you submitted to our office expires Drinr to 12/31/2015.
The Rules set out responsibilities, obligations and liabilities of the users and/or owners of the Wells within the South Platte
River Basin of the State of Colorado. In general,the Rules require that no later than December 31, 2015 all Wells within the
scope of the Rules are required to comply. In order to comply,Wells shall either:
1. Ensure a totalizing flow meter(TFM) is installed on the well, meeting the Rule's installation requirements,AND
submit a totalizing flow meter verification (form 3.1) meeting the Rule's accuracy requirements; OR
2. Ensure the well is equipped with a verified alternative method of measurement(i.e. PCC)(form 3.2 or form
11); OR
3. File the well as Inactive(form 7), ensuring that the well is Inactive in accordance with the Rules.
The forms to accomplish these things can also be found at the internet address listed above.
Totalizing flow meters must be verified as accurate prior to December 31,2015 and re-verified every four years thereafter. A
list of qualified public well testers, certified by DWR to verify totalizing flow meters in accordance with the Rules, is available
here:

Note that requirements of the Rules do not superceed a term or condition of an existing water court decree. You are required
to comply with all terms and conditions of court decrees and the Rules. Your augmentation plan administrator may have
already contacted you about the Rules requirements. If your augmentation plan will be the one to verify the totalizing flow
meter, please be aware that the well owner/user is ultimately responsible for compliance with the Rules. If a TFM verification
test has been performed recently and submitted to DWR,the verification test is valid for 4-years from the date of test and you
do not need to perform another test until expiration(indicated in the above table(Test Expiration Date).
