WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Work Order Form (Flood Repairs -
EM-BR54/13A) Pursuant to Agreement for Design and Construction Administration Services for
Permanent Repairs between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Public Works, and
Bohannan Huston, Inc., commencing April 28, 2015, with a completion date of September 30,
2015, and with further terms and conditions being as stated in said work order form, and
WHEREAS, after review, the Board deems it advisable to approve said work order form, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Work Order Form (Flood Repairs - EM-BR54/13A) Pursuant to
Agreement for Design and Construction Administration Services for Permanent Repairs between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Public Works, and Bohannan Huston, Inc., be, and

RE: Thompson River Corridor Flood Recovery Project, Design
Consultant Contract Modification regarding Bridge 54/13A
DEPARTMENT: Public Works DATE: 04/19/15
Brief description of the problem/issue:
The Public Works Department entered into a contract with Bohannah Huston Inc (BHI) for
design of permanent flood recovery repairs in August of 2015. BHI has satisfactorily completed
design work for the Thompson (FEMA) Sites Flood Recovery Project. Now the County would
like to have BHI do design work for flood recovery repairs for Bridge 54/13A, the remaining
Thompson River Corridor site, which is an FHWA site. Because Bridge 54/13A is an FHWA
site, due to the classification of WCR 54 as an arterial roadway, the more stringent design,
review and acceptance process by CDOT is required. Weld County previously received a scope
and fee proposal from BHI which included Bridge 54/13A. At that time, in August of last year,
the estimated design cost for Bridge 54/13A was less than $20,000 however a work order was
not done for 54/13A. Since that time, the County has met with FHWA and they have revised the
scope requirements for this site. Based on this direction from FHWA, BHI revised the proposed
design fees which are now $51,223.20.
Bridge locations 3/42A, 5/42A, 15.5/52, 19/46.5A, 13/54A, which were all significantly
damaged by the September 2013 floods, are now out to bid for permanent repairs. These are the
FEMA sites which had work orders done previously with BHI. Public Works believes that due
to the more stringent design, review and acceptance process by CDOT, that a separate work
order should be done for this work based on the proposal BHI has submitted which is (attached).
Staff believes the cost estimates are reasonable for the design of this work. Public Works
recommends the design contract already in place with BHI be modified to include these repairs.
What options exist for the Board? (Include consequences, impacts, costs, etc. of options)
1) Authorize staff to move forward with the attached work order to include this additional work,
or
2) Direct staff to find another solution for accomplishing this work.
Recommendation: Public Works recommends that a work order in the amount of
$51,223.20, be signed by the consultant and appropriate staff.

Based on a review of redline comments provided on an August 5, 2014 version of this scope
and fee document, meetings, and on-site visits with Weld County in association with the
project noted above; Bohannan Huston, Inc. (BHI) appreciates the opportunity to submit
this response to the Design Services Work Orders request from the County for bridge and
roadway related repairs on the Big and Little Thompson drainageways.
Based on our previous work for :he County on the Dry Creek drainages, and a review of the
sites and scope anticipated by the County; we have developed the attached, scope of
services, fees, and schedule. The following Exhibits are included with this letter:
A. Scope of Services
BHI has prepared a detailed Engineer's Scope of Services (Exhibit A). These items are
based on our current understanding of the project, and we welcome the opportunity
to discuss the scope herein if it does not meet your expectations. Specific scope
items for each site are listed on the Fee Estimate described below.
B. Fee Breakdown
BHI has prepared a detailed Hourly Breakdown and Cost Estimate (Exhibit B)
detailing hours for each team member including Direct Costs from subconsultants
and reimbursable expenses.

Engineer's Scope of Services
Weld County Flood Recovery—Big and Little Thompson Corridors
EM-BR 54-13A
BASIC SERVICES
The Engineer's (Bohannan Huston, Inc. or BHI) and sub-consultants Basic Services shall consist of the items
described below. We understand that it is Weld County's intent to prepare construction plans for each site
to meet the original, pre-flood conditions.
The following scope of services is anticipated for site EM-BR54-13A as detailed in Exhibit B.
1. Land Survey Services
1.1 Field Verification of Previous Survey:
a. Obtain any County and State permits that are required to complete the necessary field work
within the existing roadway rights-of-way and within adjacent private property.
b. Conduct a field survey to obtain select topographic data to supplement and verify the County
supplied topographic data from the project site prepared by others.
c. Contract with an independent subcontractor to perform a surface locate of existing utility lines
where necessary within the project area and conduct a field survey to locate the marked
locations.
d. The location of identifiable visible surface evidence of the various utility systems, such as utility
poles, manholes, and culverts will also be shown. Information shown on utility records that are
available at the time of the survey shall be incorporated into the survey. The following will be
located and obtained for specific visible utilities where access is possible:
• Water systems:surface location of all valves and hydrants.
• Dry Utilities: location of poles,valves, pedestals and transformers.
• Other visible utility surface features.
• The alignment of non-visible underground utilities will be based upon the location of
surface markings made by an underground locating service or the utility companies.
e. Prepare a base file that combines the topographic survey and the field survey to be used in the
engineering design with utility information and any other surface evidence of buried or
overhead utilities found while onsite.

a. CDOT Right-of-Way Plan Set:
Presurvev Conference
• Meeting will be held with Weld County and CDOT Region 4.
Survey Control
• Utilize CHARH and/or Weld County Published control to establish a site control network.
• Set three CDOT Standard Type 6 survey control monuments within the project limits.
• Survey control monuments within the project limits will be set in accordance with the CDOT Survey
Manual requirement for Topographic and Right-of-Way Surveys.
Ownership Mapping
• The purpose of the ownership map is to show plan sheet information as it relates to total contiguous
ownership. The ownership map will identify conflicts in property boundaries as result of the field
survey. For this project, two parcels that are abutting or contiguous to the existing highway Rights-
of-Way will be included in the ownership map.
• The ownership map will contain: title block, revision block, north arrow, graphic scale, sections,
townships, range, Principal Meridian, centerline orientation, a scale drawing of the boundaries of all
land ownerships affected by the highway improvement, recorded and apparent rights-of-way and
easements identified by title commitment reports prepared by others, ownerships will be numbered
alternately as they occur from centerline from south to north or west to east in the same direction
as the stationing to correspond with the plan sheets, current names of owners and lessees, the
ownership map will be drawn in accordance with specifications; normal scale, 1"=400' in rural areas,
and 1:2,000 in urban areas. If the entire ownership will not fit on the sheet at this scale, an
additional abbreviated OWNERSHIP MAP may be used at a scale of 1:50,000, or other suitable scale,
to show the configuration of large ownerships. If BHI encounters situations where a smaller scale
might be necessary in order to include total ownerships we will prepare small scale supplemental
sheets solely to show the total ownerships. The right-of-way alignment will be shown but not
dimensioned except for centerline bearings, important ties, and right-of-way widths, curve data may
be abbreviated to show the delta angle, right or left, and the degree of curve, beginning and ending
stations of the right-of-way project and stationing at 200 foot intervals. If possible, deed bearings
and distances on property lines outside the rights-of-way will be shown, improvements and
topography within the ownerships as well as existing access to the street system, structures,fences,
rivers, streams, and drainages. Field roads and topography will be included to identify their effect on
access and access control. When access is controlled, approved overpasses, underpasses, grade
separations, interchanges, grade intersections, public and private access points will be shown.
Natural water courses, canals, lakes, large reservoirs, culverts five feet and larger (designed
primarily for drainage and bridges),will be shown with their size indicated. Railroads,jeep trails and
frontage or service roads, planned to be constructed, will be shown and the owner's or lessee's
system of cross fencing will be indicated as an aid to understanding operating methods.

Right-of-Way Plans
• The Right-of-Way (R.O.W.) plans will show the location of the existing R.O.W. and Temporary
Easements. We have identified two parcels that are abutting or contiguous to the highway
Rights-of-Way that will require Temporary Easements.
• The final R.O.W. Plans will show:
(As applicable) R.O.W and access control line, width to be acquired, proposed slope limits,
centerline and stationing with appropriate lines and changes in R.O.W. widths, any additional
easement areas, either temporary or permanent, that are required to accommodate
intersecting roads and streets, land service, access and temporary roads, drainage areas,
material storage areas,slope widening, utilities, railroads, or any other special uses, all pertinent
data affecting the cost of the R.O.W. such as structures, land service or access roads,
improvements, drain fields anc fences, all approved points of entry to or exit from the traffic
lanes, even where the R.O.W. lines and access control lines are coincident, disposition of
improvements within the proposed R.O.W., parcel identification number, the property
ownership lines,the name of the property owners,the area in acres or square feet of the part to
be taken and of each remainder of a partial taking, the size, form, and arrangement of R.O.W.
Plans to conform to the general requirements for highway plans and should contain sufficient
dimensional and angular data to permit ready identification and correlation with the legal
descriptions of all parcels, easements and special use areas that are required by the associated
highway project.
State Requirement (CRS 38-51-106)
All land survey plats shall include but shall not be limited to the following:
A scale drawing of the boundaries of the land parcel, all recorded and apparent rights-of-way
and easements, and, if research for recorded rights-of-way and easements is done by someone
other than the professional land surveyor who prepares the plat, the source from which such
recorded rights-of-way and easements were obtained; or if the client wishes not to show rights-
of-way and easements on the land survey plat, a statement that such client did not want rights-
of-way and easements shown, all field-measured dimensions necessary to establish the
boundaries on the ground and all dimensions for newly created parcels necessary to establish
the boundaries on the ground, a statement by the professional land surveyor that the survey
was performed by such surveyor or under such surveyor's responsible charge, a statement by
the professional land surveyor explaining how bearings, if used, were determined, a description
of all monuments, both found and set, that mark the boundaries of the property and of all
control monuments used in conducting the survey. If any such boundary monument or control
monument marks the location of a lost or obliterated public land survey monument that was
restored as a part of the survey on which the plat is based, the professional land surveyor shall
briefly describe the evidence and the procedure used for such restoration. If any such boundary
monument or control monument marks the location of a quarter section corner or sixteenth
section corner that was established as a part of the survey, the professional land surveyor shall
briefly describe the evidence and procedure used for such establishment, unless the corner
location was established by the mathematical procedure as outlined in section 38-51-103,a
statement of the scale or representative fraction of the drawing, and a bar-type or graphical
scale, a north arrow, a written property description, which shall include but shall not be limited

to a reference to the county and state together with the section, township, range, and principal
meridian or established subdivision, block and lot number, or any other method of describing
the land as established by the general land office or bureau of land management, the signature
and seal of the professional land surveyor, any conflicting boundary evidence and a statement
defining the lineal units used including but not limited to meters, chains, feet, and U.S. survey
feet. If it is necessary to define conversion factors,the factors shall be a function of the meter as
defined by the United States department of commerce, national institute of standards and
technology.
CDOT Plan Set Requirements
R.O.W. Plans will be in accordance with the Memorandum of Understanding between the
Colorado State Board of Licensure for Professional Engineers and Land Surveyors and CDOT and
in accordance the applicable laws of the State of Colorado. Because ROW Plans will be deposited
with the respective counties per State Statutes, the dimensions and type of material used for
the plan sheets must comply with the law.
1)A"Title Sheet", numbered sheet number 1.01.
2) One or more "Tabulation of Properties" sheets numbered numerically beginning with Sheet
2.01 followed by sheet numbers 2.02, 2.03,etc.
3) One or more "Project Control Diagram" sheets as determined by the Region Survey
Coordinator, numbered numerically beginning with Sheet 3.01,followed by sheet numbers 3.02,
3.03,etc.
4) One or more "Land Survey Control Diagram" sheets as determined by the Region Survey
Coordinator, numbered numerically beginning with Sheet 4.01,followed by sheet numbers 4.02,
4.03,etc.
5) One or more optional " Monumentation Sheet " sheets, numbered numerically beginning
with Sheet 5.01 and followed by sheet numbers 5.02, 5.03 etc. .
6) One or more " Tabulation of Road Approach " sheets numbered numerically beginning with
Sheet No. 6.01 followed by sheet numbers 6.02, 6.03 etc. Tabulation of Road Approach Sheets
are optional depending on the project type.
7) One or more " Plan Sheet " sheets numbered numerically beginning with sheet number 7.01
followed by sheets 7.02, 7.03 etc. Details too large or too complicated to be shown on a plan
sheet,should be drawn on a separate plan sheet numbered numerically.
8) One or more "Ownership Map" sheets numbered numerically beginning with sheet number
8.01 followed by sheets 8.02, 8.03 etc.
R.O.W. Plan Review
1) The Region R.O.W. Designer willll hold a R.O.W. Plans Review (ROWPR) with the Region
R.O.W. Manager and others as deemed necessary immediately preceding authorization to
appraise and acquire. The Region R.O.W. Designer or Surveyor shall determine if the R.O.W.
Plans are sufficiently complete in accordance with this manual and CDOT"s R.O.W. Plans Review
Guidelines.
2) R.O.W. Plans will not be submitted to the Central Office Project Development Branch, R.O.W.
Services for authorization until after the ROWPR has been held and all parties agree as to the
R.O.W.to being acquired.

3) Once a project has been authorized for acquisition, design revisions which affect R.O.W. will
not be considered until the revision has been justified by the office initiating the revision and
concurred in by the Region Office Survey Coordinator or the Region R.O.W. Manager and the
Region Preconstruction Engineer.
4) Once a parcel has been filed for condemnation with the Attorney General's Office, R.O.W.
Plan revisions will not be considered until they have been approved by the Chief Engineer.
Unless the landowner agrees to the proposed revisions, a motion to amend the legal description
must be filed with the court.The granting of such a motion is discretionary with the court.
Recording of the R.O.W. Plans
Once the R.O.W. Plans for the oroject are completed and monuments set in the field, the final
R.O.W. Plans shall be signed and sealed by the Colorado Professional Land Surveyor(s) licensed
in the State of Colorado who is (are) in responsible charge for the R.O.W. Plans stating that the
R.O.W. Plans and legal descriptions were prepared under his/her direct supervision and
checking, and by the Professional Land Surveyor(s) licensed in the State of Colorado who is (are)
in responsible charge for setting the monumentation stating that the monumentation was set
under his/her direct supervision and checking. Current CDOT drafting standards call for multiple
plan sheets to be placed on the appropriate sized page (24x36" or 18)(24") depending on the
county(ies) involved. The final signed and sealed R.O.W. Plans will be deposited in the
appropriate County Clerk and Recorder's office within six months after the monumentation was
set in the field in accordance with the Memorandum of Understanding between the Colorado
State Board of Professional Engineers and Land Surveyors and CDOT, and Colorado State Law.
A complete, non-colored, 11"x17" Right-of-Way Plan Set, comprising all sheets contained the
deposited plan and bearing the Professional Land Surveyors" original signatures and seals and
depositing information shall be sent to the Central Office Project Development Branch, R.O.W.
Services along with a memo staring the following information:
1) Region

5)Highway Route Number and Segment
6)Beginning and ending mile post
7)Counties
8)Sections,Townships, Ranges and Principal Meridian

Once the memo along with a copy of the final deposited R.O.W. Plans are received at Central
Office Project Development Branch, R.O.W. Services the title sheet shall be stamped by R.O.W.
services and filed with the project records as follows:

Temporary Easements
• Temporary Easements will be shown on the R.O.W. plan set. Separate document legal
descriptions will not be prepared as part of this scope of work. As described in the CDOT R.O.W.
Manual, should condemnation be required, separate document legal descriptions will be
prepared as an additional service.
2. Improvements Back to Pre-Flood Conditions
2.1 Kickoff and Site Meeting:
BHI will visit the site along with the County PM at the existing bridge crossings and surrounding
areas in order to make an assessment as to the overall condition of the structure, roadway, and
surrounding drainage features; and determine the general scope of work and survey limits required
for each structure location. We will identify the bridge and drainage components that were
damaged due to the September 2013 flooding and make recommendations for repair to pre-flood
conditions.
2.2 Project Management:
Provide overall project management including invoicing, meeting minutes, and coordination
between the County and the project sub-consultants.
2.3 Grading and Seeding:
BHI will prepare design documentation to facilitate earthwork construction for areas affected by the
flooding. Generally this work includes the return of roadway embankments to pre-flood conditions,
grading and dressing areas around the channel, minor silt removal, and areas of bank protection
armoring. Areas disturbed by grading activities and/or areas disturbed by previous emergency
construction will be indicated to be seeded. Grading plans will be prepared depicting contours to
reestablish channel banks and roadway embankments to pre-flood conditions.
2.4 Placement of Bank Protection:
Channel reconstructions including riprap bank protection placement details will be prepared. Bank
treatment designed to minimize future scour and erosion around bridge and roadway
embankments, and return the channel to the pre-flood condition. This task include removal of
rubble on west abutment and replace with riprap.
2.5 Replace Roadway Patches:
Prepare plans for roadway repairs near east abutment.
2.6 SWMP report and Erosion Control Plans:
Erosion control plans will be prepared showing erosion control blanket locations. In seeded areas,
erosion control blankets will be indicated to establish and protect seeding. An erosion control plan
will be prepared to address the erosion and sediment control of each site during construction. Plans
will also show areas of debris removal. This task includes placement ofTRM along 800 ft of road on
the south side west of the bridge.

2.7 Hydrology and Hydraulics Report:
BHI will prepare a preliminary Hydraulic Design Report tailored to satisfy the specific requirements
of this project. The preliminary report will contain hydrologic and hydraulic data, pertinent
information taken from the field survey of the project area,applicable project photographs,
calculations,and a drainage plan.
2.8 Plans, Specifications, and Estimate:
a. BHI will prepare construction plans for the work at the bridge crossing location. An
abbreviated set of plans will be prepared incorporating the surveyed topography,
existing contours, and ROW information. Plans will include the following if required
for each site:
• Cover/Location Sheet
• Typical Sections
• General Notes
• Survey Control Diagram
• Plan and Profile Sheets
• Grading and Drainage Sheets
• Erosion Control Plan/SWMP
• Detail Sheets
b. BHI will calculate and summarize estimated quantities in a Bid Schedule.
c. BHI will prepare a construction cost estimate using the CDOT "Item Description and
Abbreviations Book"for proper descriptions and code numbers.

2.9 Environmental:
CDOT CAT X Form Completion includes meeting with CDOT to confirm issue clearances; includes
burrowing owl survey,wetlands, migratory birds,threatened and endangered species.
2.10 Title Commitments and Easement Acquisitions
Subconsultant H.C. Peck and Associates, Inc. will obtain title commitments and coordinate with the
County to determine easement value, prepare and required documentation, meet with property
owner to present offer package and to secure owners signature, provide informal closing with the
property owner, and transmit documents to Weld County for ROW Clearance.
3. Bidding and Construction
3.1 Construction Inspection
a. The scope of work will be for a full time construction inspector who will be responsible for the
day to day activities assigned by the County Project Manager; this includes inspecting and
documenting all construction activities, and preparation of daily diaries documenting
construction activities and relevant observations in accordance with CDOT and Weld County
standards.

b. Attendance at the pre-bid meeting. BHI will attend the pre-bid conference and prepare
presentation materials as requested by the County.
c. Attend a pre-construction conference in coordination with the key personnel.
d. In order to provide a consistent documentation platform,we will use County supplied forms for all
material submittals/shop drawings, Requests for Information (RFI), Design Revisions (DR),daily field
reports,field and materials test reports,weekly meeting minutes, change orders and pay
applications, cost proposals,and other related documentation.
e. Review shop drawings and submittals and attend weekly construction progress meetings.
f. Provide one full time Construction Inspector to inspect the work of the contractor and assist the
contractor during the time the construction is in progress to reasonably confirm if the project is
being constructed in general conformance with the plans and specifications.
g. Review field and materials test reports for compliance with the contract documents. BHI will
monitor the field test reports and notify the construction contractor of any failed tests as soon as
they are discovered.
h. Provide weekly construction inspection reports documenting daily weather conditions, work in
progress and location, equipment in use (types and quantity), size of work force, discussions of
problems encountered and resolutions, hours worked per day, materials delivered and installed,
instruction given to contractors, principal visitors, etc.
i. Process Requests for Information (RFI)and Design Revisions(DR). The BHI design engineer will
review and respond to all RI's and DRs.
j. BHI will review cost proposals, provide recommendations to Owner, and certify field change orders.
Our PM and Construction Inspector will meet with the construction contractor to verify change
order quantities. Excludes negotiations/coordination associated Pay Reductions for Non-Complying
In-Place Materials and Engineer's Recommendation Letter.
3.1 Construction Materials Testing
BHI's trusted subconsultant, Geocal, Inc. will provide Construction Materials Testing for the project,
including a Field Materials Technician and including test for Standard Proctor (Embankment),
Gradation,Atterberg Limits, and aggregate specific gravity (riprap). Asphalt lab tests will include AC
Content,Gradation,and maximum specific gravity.
3.3 CDOT Closeout/As-Builts.
a. Conduct a final inspection of the public work with the County, contractor,and other
entities/agencies and provide a recommendation regarding acceptance,with all CDOT closeout and
record drawings.
