RE: APPROVE TWO (2) PURCHASE OF SERVICES AGREEMENT AMENDMENTS WITH
VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two (2) Purchase of Services
Agreement Amendments between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Area Agency on Agency, and the various providers listed below, commencing May 1,
2015, and ending June 30, 2015, with further terms and conditions being as stated in said
agreements, and
Meals on Wheels
Senior Resource Services
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the two (2) Purchase of Services Agreement Amendments
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Agency, and the various providers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.

RE: Amendments to Purchase of Service greements between
the Weld County Department of Human Services' Area
Agency on Aging and Various Providers
Enclosed for Board approval are Amendments to Purchase of Service Agreements between the
Departments' Area Agency on Aging and Various Providers. These amendments were reviewed
under the Board's Pass-Around Memorandum dated May 8, 2015, and approved for placement
on the Board' s Agenda.
The Weld County Area Agency on Aging is requesting permission to increase the FY 15 grant
amounts of two providers. These funds were initially allocated to another provider but due to
staffing issues with that provider, they will not be able to expend the funds by June 30, 2015 .
These available state funds cannot be carried over from one fiscal year to another. The major
provisions are as follows:
a. Meals on Wheels — increase from $ 130,000 to $ 170,000 ($5 .73 per meal
reimbursement). Total number of meals served to homebound seniors with the
increased funding would be 29,668; previously 22,687.
b. Senior Resource Services — increase from $50,000 to $65,000 ($ 11 .98 trip
reimbursement). Total number of trips provided to seniors with the increased
funding would be 5,426; previously 4, 173 .

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of May,2015 by and between the Weld County Department
of Human Services' Area Agency on Aging,hereinafter referred to as "AAA" and Meals on Wheels, hereinafter
referred to as "MOW"
WITNESSETJI:
WHEREAS,the "AAA" desires to continue to purchase adult day services from"MOW", and
WHEREAS,the "AAA" and"MOW" are desirous of amending the original agreement of July 1, 2014;
NOW THEREFORE, it is mutually agreed:
I. The "AAA" will increase the current contract from i 130,000 to $170,000,reimbursing"MOW" $5.73 per meal
served. "MOW" agrees to provide a total of 29,668 units of services for eligible homebound seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from May 1,2015 through June 30, 2015. This Amendment will be terminated
immediately in the event of loss of funding.

of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and Senior Resource Services, hereinafter
referred to as"SRS"
W ITNESSE.II I:
WHEREAS. the "AAA" desires to continue to purchase adult dac services from "MOW'. and
WI IEREAS, the "AAA" and "SRS"are desirous of amending the original agreement of July I, 2014:
NOW THEREFORE, it is mutually agreed:
I. The "AAA" will increase the current contract from i 50,000 to 565,000. reimbursing"SRS" $11.98 per one—way
ride served. "SRS' agrees to provide a total of 5,426 units of services for eligible seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from May I,2015 through June 30,2015. This Amendment will be terminated
immediately in the event of loss of funding.
