RE: APPROVE STANDARD FORM FOR CHILD WELFARE CONTRACT AGREEMENT
EXTENSION/RENEWAL BETWEEN THE WELD COUNTY DEPARTMENT OF
HUMAN SERVICES AND VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
AGREEMENTS CONSISTENT WITH SAID FORM
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board, sitting as the Weld County Board of Human Services, has been
presented with the form of a Child Welfare Contract Agreement Extension/Renewal between the
Weld County Department of Human Services and various providers, and
WHEREAS, after review, the Board deems it advisable to approve the form of said
Extension/Renewal, a copy of which is attached hereto and incorporated herein by reference,
and to delegate standing authority to the Chair of the Board of County Commissioners to
execute individual agreements between the Department of Human Services and various
providers.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, sitting as the Weld County Board of Human Services, that the form
Child Welfare Contract Agreement Extension Renewal between the Weld County Department of
Human Services and various Child Welfare providers be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign any agreements consistent with said form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of February, A.D., 2015.

Agreement Extension/Renewal Template
Enclosed for Board approval is the Department of Human Services Child Welfare Contract
Agreement Extension/Renewal Template. This agreement template was reviewed under the
Board's Pass-Around Memorandum dated February 5, 2015, and was approved for placement on
the Board's Agenda.
During discussions this past year with County Legal and Purchasing,it was determined that
Child Welfare could renew existing approved agreements for up to two years post initial
agreement, allowing for a maximum of three agreement terms before a new bid proposal from
the provider would be required. Through collaboration with Legal, the template was developed as
a means of executing an annual renewal (1 year timeframe only). A renewal will only occur if the
provider is in good standing with the County, has consistently met the requirements of their
existing contract, is providing a service the Department continues to need, and is in agreement
with renewing their contract agreement without substantial changes to their existing agreement.
Per Legal, minor changes are permitted to scope and rate, but any substantial changes would
require the provider to submit a new bid proposal. The renewal document ensures that the
original agreement is clearly identified, highlights any changes and outlines the term of the
renewal. Unless otherwise determined by Legal, the original agreement will be submitted with
the renewal for reference.
