HomeMy WebLinkAbout20153052.tiff RESOLUTION RE: APPROVE MEMORANDUM OF UNDERSTANDING FOR THE COLORADO HEALTH FOUNDATION (TCHF) AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Memorandum of Understanding for The Colorado Health Foundation (TCHF) between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Pikes Peak Area Council of Governments, and Colorado Association of Area Agencies on Aging, commencing July 1, 2015, and ending June 30, 2016, with further terms and conditions being as stated in said Memorandum of Understanding, and WHEREAS, after review, the Board deems it advisable to approve said Memorandum of Understanding, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Memorandum of Understanding for The Colorado Health Foundation (TCHF) between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Pikes Peak Area Council of Governments, and Colorado Association of Area Agencies on Aging, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair Pro-Tem be, and hereby is, authorized to sign said Memorandum of Understanding. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 21st day of September, A.D., 2015, nunc pro tunc, July 1, 2015. Request Board Approval for the Departments' Area Agency on Aging(WCDHS AAA) Memorandum of Understanding(MOU)with the Pikes Peak Area Council of Governments and the Colorado Association of Area Agencies on Aging regarding The Colorado Health Foundation (TCHF)grant. This MOU sets forth the roles and responsibilities of each party in the use of funds granted by TGIF. These funds are specifically granted to support the network of Aging and Disability Resources for Colorado(ADRC) program sites, as the structure of Long-Term Services and Supports(LTSS) in Colorado is redefined by state leadership in the Departments of I Iealth Care Policy& Financing and Human Services. The term of this MOU is from July 1, 2105 through June 30, 2016. Pikes Peak Area Agency on Aging is acting as the Fiscal Agent of the grant in order to distribute the grant dollars. WCDHS AAA will be provided a total grant amount of$44,000. t do not recommend a Work Session. I recommend approval of this MOU. Approve Request BOCC Aecnda Work Session Sean Conway Steve Moreno l Barbara Kirkmevcr Mike Freeman Julie Cozad • Pass-Around Memorandum; September 3, 2015 -Contract ID 183 Page 1 2015-3052 Memorandum of Understanding Between the Pikes Peak Area Council of Governments, the Colorado Association of Area Agencies on Aging and the Weld County Department of Human Services Area Agency on Aging, by and through the Weld County Board of Commissioners regarding the Colorado Health Foundation grant funds Parties to this agreement The parties to this memorandum are the Pikes Peak Area Council of Governments(PPACG),the Colorado Association of Area Agencies on Aging (C4A) and the Weld County Department of Human Services Area Agency on Aging, by and through the Weld County Board of Commissioners(WCDHS AAA). II. Purpose The purpose of this Memorandum of Understanding (MOU) is to set forth the roles and responsibilities of each party in the use of funds granted by the Colorado Health Fou dation (TCHF).These funds are specifically granted to support the network of Aging and Disability Resources for Colorado (ADRC) program sites, as the structure of long-term services and supports(LTSS) in Colorado is redefined by state leadership in the Departments of Health Care Policy& Financing and Human Services. The Colorado Health Foundation's intent is to help create a streamlined and sustainable entry point system, making it easier for Coloradans to access LTSS. III. Terms The term of this MOU is from July 1, 2015 through June 30, 2016. IV. Summary of the Aging and Disability Resources for Colorado(ADRC)program The Aging and Disability Resources for Colorado (ADRC) program, provides streamlined access to long-term services and supports (LTSS)to seniors (adults 60 and older) and adults living with disabilities, through both Information and Referral services and Options Counseling. These services and supports are provided with the goal of helping consumers to live as independently as possible, while remaining in their communities. Information and Referral services are provided state-wide through all of the Area Agencies on Aging (AAAs), as well as the Colorado 2-1-1 network. Options Counseling, a consumer-directed form of short-term case management, is provided through the ADRC program sites to those consumers who are unable to access the resources available in their region without assistance. The goal of the ADRC program is to use Information and Referral services and Options Counseling to enable the service providers and the consumers to work together to best fit the services required to meet the needs of the consumer. Using a consumer-directed multipronged approach, consumers who are able to navigate long-term care systems on their own are given the information they need, while those who require more involvement by an options counselor, may be helped every step of the way. V. Summary of Colorado Health Foundation grant award The Colorado Health Foundation (TCHF)grant award to the Colorado Association of Area Agencies on Aging (C4A) with the Pikes Peak Area Council of Governments as the fiscal agent is$704,000 over 12 months,to provide streamlined access to long-term services and supports (LTSS). Of that total granted amount, $44,000 will be provided to WCDHS AAA.This grant is considered by TCHF to be a bridge grant, intended to help the ADRC sites transition from TCHF financial support provided over the past two years,to sustainable long-term funding. VI. Statement of Work and Responsibilities Beginning July 1, 2015 through June 30, 2016, duties for WCDHS AAA under the terms of this MOU will include, but will not be limited to: • Participate in and support the commitment of the ADRC network state-wide to report to the State Unit on Aging the individuals from whom information/referral contacts were received and unduplicated options counseling service units were provided. • Participate in and support the commitment of the ADRC network state-wide to report to the State Unit on Aging the individuals who were enrolled in Medicaid LTSS. • Continue to Incorporate and strengthen strategies to better integrate the disability community with ADRC practices. o Maintain updated resources with the AAA, ILC and CCB in their area to confirm the accuracy of referrals. o Partner with local AAA, ILC and CCB to maintain an outreach strategy which informs eligible consumers about ADRC services. o Work with local AAA, ILC and CCB to prevent duplication of options counseling services in the region. • Collaborate in the final evaluation and report, which will inform the final grant report to the Colorado Health Foundation. By signing this MOU, the parties agree to abide by the terms listed herein for ADRC services provided under the Colorado Health Foundation funding to the Colorado Association of Area Agencies on Aging, with Pikes Peak Area Council of Governments as the fiscal agent.
