Dear Ms. Mika:
In reviewing our files, your contract with the Department of Local Affairs will expire on July 31,
2015. Per the contract conditions, a final report is due in order to process your final payment.
Please file that report and all payment requests with your Regional Manager. All expenses
claimed for reimbursement from grant funds must be incurred by the contract termination date.
If you are aware at this time that you will be unable to complete the project, please notify your
Regional Manager as soon as possible. In many cases, a contract amendment to extend the term
can be executed with the state. The amendment process may take as much as 30 days to
complete, so your prompt consideration and handling of this matter is appreciated.
If you have any questions, or foresee any difficulty in completing the project, please contact Don

for further assistance. If you have already completed the project prior to this letter, please
