Extension/Renewal with Various Providers.The below parties agree to extend the term of their
original agreement, in accordance with the conditions of the original agreement,under the standard

AND SAVIO(CORE)
This Agreement Extension/Renewal("Renewal"),made and entered intO day of J�G ,2015,by and between
the Board of Weld County Commissioners,on behalf of the Weld County Department of Human Services, hereinafter referred to as
the"Department",and Savlo,hereinafter referred to as the"Contractor".
WHEREAS the parties entered Into an agreement(the"Original Agreement")identified by the Weld County Clerk to the
Board of County Commissioners as Document Number 2014-3046,approved on 10/06/2014.
WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the
Original Agreement,which Is Incorporated by reference herein,as well as the terms provided herein.
NOW THEREFORE, In consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on May 31,2015.
• The parties agree to extend the Original Agreement for an additional one(1)year period,which will begin June 1,201.5,and

Request Board Approval for the Departments' Child Protection Agreement
Extension/Renewal with Perklen Center for Psychotherapy(Core). The parties agree to
extend the term oftheir original agreement(2014-3046), in accordance with the scope of the
original agreement. under the standard km-in for renewals. The standard form was approved as
new business on March 9, 2015. The renewal term is tier June I, 2015 to'May 31. 2016.
Under this renewal, the parties agree to rate structure changes. These are identified in Exhibit D
