Subject: Acceptance of Weld County Collaborative Management Program MOU for SFY 2015-16
Dear Director Griego,
The purpose of this letter is to communicate to you information about your county's participation in the
Collaborative Management Program (CMP), 24-1.9.101, also known as HB 04-1451.
The Weld County CMP Memorandum of Understanding (MOU)for SFY 2015-16 is accepted. Your county has
elected to participate in surplus distribution. Weld County has not requested any waivers.
As specified in the MOU, the Weld County CMP plans to serve a minimum of:
A) 415 children and/or youth through designated Integrated Service and Support Team (ISST) or
multi-system involved teams.
B) 1070 children and/or youth served through CMP funded prevention programs.
The Colorado Department of Human Services, Division of Child Welfare appreciates Weld County's
continued commitment to your coordination and integration of services to children, youth and families in
your community.
