Protection Agreement Extension/Renewal with Various
Providers
Please review and indicate if you would like a work session prior to placing these items on the Board's
agenda.
Request Board Approval for the Departments'Child Protection Agreement Extension/Renewal with
Various Providers.The below parties agree to extend the term of their original agreement,in accordance
with the conditions of the original agreement,under the standard form for renewals.The standard form was
approved as new business on March 9,2015.The renewal term is for June I,2015 to May 31,2016 for the
following contracts and contractors:

NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on June 3O 2015.
• The parties agree to extend the Original Agreement for an additional one(1)Year period,which will begin
will end on June 30.2016.
• The Renewal,together with the Original Agreement,constitutes the entire understanding between the parties. The
