RE: APPROVE FATIGUE RISK MANAGEMENT HEALTH AND SAFETY STRATEGY AND
AUTHORIZE CHAIR TO SIGN - 3:1 CORPORATE HPM SOLUTIONS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Health and Safety Strategy between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Resources, and 3:1 Corporate HPM Solutions,
commencing March 25, 2015, upon execution of signature, for a period of six (6) months, with
further terms and conditions being as stated in said strategy, and
WHEREAS, after review, the Board deems it advisable to approve said strategy, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Fatigue Risk Management Health and Safety Strategy between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Resources, and 3:1 Corporate HPM Solutions be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said strategy.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 6th day of April, A.D., 2015, nunc pro tunc March 25, 2015.

RATIONALE
There is no question that sleep apnea is a dangerous and growing problem for the
U.S. transportation industry. It is inextricably related to the epidemic of obesity,
while being associated with daytime sleepiness and fatigue, and significant " '' - '°l
morbidity and mortality due to accidents and cardiovascular events. Obesity and "'"7""+-
related sleep apnea are challenging to both individuals and organizations — contributing to worker fatigue, and
associated health and safety risks and reduced productivity. Consequently obesity related fatigue is being studied in
an attempt to minimize risks and maximize productivity. Consider this:
• Data show that about 20 percent of commercial drivers may have obstructive sleep apnea. Source: AASM:

• A recent Gallup-Healthways Well-Being Index found transportation workers to have the highest obesity rate,
37.8%, of any U.S. industry and the corresponding cost of lost productivity due to absenteeism to be 3.5

Workers who are obese have significantly shorter endurance times (40%) when performing workplace tasks,

➢ Studies also indicate that comprehensive worksite health promotion programs play an important role in
fatigue risk management by helpine change unhealthy lifestyle habits of workers and positively impacting
health and safety risk factors related to fatigue. In fact, a strong body of research indicates that programs
focusing on lifestyle changes exceed the efforts of addressing hours worked and optimizing working
conditions.
The recent evolution of wearable devices, particularly those that monitor physiology (heart rate,
temperature, SpO2), coupled with activity and movement, bring biofeedback monitoring and associated
behavior change into the workplace further enhancing health promotion efforts. Biofeedback has been found
to be useful in assisting workers with:
1. Understanding the importance that lifestyle and external events have on sleep-wake cycles and
fatigue levels at work
2. Changing sleep hygiene and lifestyle habits (healthy weight, activity and stress management) which
affect fatigue levels
Current global best practices for implementing fatigue risk management programs consist of 7 key
characteristics which consequently mirrnr the 7 henrhmarka of recnItc-nriented wnrksite health promotion
programs:
1. Science based—Supported by established peer-reviewed science
2. Data driven—Decisions based on collection and objective analysis of data
3. Cooperative— Designed together by all stakeholders
4. Fully Implemented—System-wide use of tools, systems, policies, procedures
5. Integrated -- Built into the corporate safety & health management systems
6. Continuously improved— Progressively reduces risk using feedback, evaluation & modification
7. Owned—Responsibility accepted by senior corporate leadership
Given the above, a comprehensive fatigue risk management program designed to manage fatigue associated health
and safety risks by optimizing the working environment (shift schedules, conditions) and promoting healthy lifestyle
choices (sleep hygiene, activity, nutrition, weight control and stress management) will benefit both workers and their
employers.

3:1 Corporate HPM Solutions serves our customers by assisting them with integrating evidence
based solutions, which enhance the health and productivity of their workforce. We are pleased to submit this
proposal for services to support Weld County's efforts towards enhancing worker health and safety while managing
and mitigating the associated risks of obesity and excessive fatigue.
PURPOSE:
Integrate wearable technology and biofeedback behavioral change programs into existing 2015 Weld County Safety
and Wellness programs as a means to manage and mitigate worker fatigue and associated health and safety risks.
GOALS:
I. To assess the sleep habits of Weld County Public Works employees and implement behavior change programs for
improvement.
Objective:
a. Reduce worker fatigue by improving worker sleep in the following areas:
1. Total sleep quality (%)
2. Time spent in deep sleep
3. Total hours spent sleeping
Metric:Compare sleep data gathered from the Withings Pulse o2 pre and post program.
II. To assess the activity habits of Weld County Public Works employees and implement behavior change programs
for improvement.
Objective:
a. Increase total number of daily steps taken by employees
Metric:Compare activity data gathered from the Withings Pulse o2 pre and post program.
III. To assess the BMI of Weld County Public Works employees and implement behavior change programs for
improvement.
Objective:
a, Reduce BMI in overweight employees, and maintain BMI in healthy weight employees.
Metric:Compare BMI gathered pre and post program.
IV. To assess the heart health of Weld County Public Works employees and implement behavior change programs for
improvement.
Objective:
a. Reduce blood pressure of Weld County Public Works employees and implement behavior change
programs for improvement.
Metric:Compare blood pressure gathered from pre and post program screenings.

EXECUTION
Our execution strategy incorporates proven biofeedback and health promotion methodologies,vetted wearable
technology by Withings, and a highly responsive approach to managing deliverables. Following is a description of our
project methods, including how the project will be developed,a proposed timeline of events, and reasons for why we
suggest developing the project as described.
I Historically biofeedback instruments were used to assist individuals with gaining greater awareness of many
physiological functions to improve health, performance, and the physiological changes that occur in conjunction with
Ichanges to thoughts,emotions, and behavior.As previously mentioned, with the advent of wearable technology
formerly expensive and prohibitive biofeedback devices/methods can now benefit anyone desiring to change a
health or performance behavior.

PERIOD OF PERFORMANCE
The period of performance for this project is six months,commencing on March 25, 2015.
FEES TOTAL:
Factoring the period of performance and the flat fees detailed above,3:1 Corporate HPM Solutions determines the
total price for the project to be $10,100 excluding any other direct costs.
TERMS AND CONDITIONS:
• Payment at time of receipt of this finalized proposal. *Other direct costs, billed monthly as accrued.
• Payment terms are Net 30 from the date of 3:1 Corporate HPM Solutions' Invoice.
• Late payments accrue a 5%per month late fee.
ASSUMPTIONS:
• 3:1 Corporate HPM Solutions will deliver services as outlined in this proposal.
