RE: Weld County Department of Human Services' Child
Protection Agreement Extension/Renewal with
Polaris Partners, LLC.
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval for the Departments' Child Protection Agreement
Extension/Renewal with Polaris Partners,LLC(Core).The parties agree to extend the term
of their original agreement(2015-0493), in accordance with the scope of the original agreement,
under the standard form for renewals.The standard form was approved as new business on
March 9,2015. The renewal term is for June I,2015 to May 31, 2016.
Under this renewal,the parties agree to rate structure changes. These are identified in Exhibit C
—Payment Schedule and Paragraph 2—Fees for Services:
• $1,700.00/Month(Therapist and Facilitator,Minimum 12 face-to-face contact hours per
month)
• $900.00/Month(Therapist only, Weekly individual and family therapy, Minimum 6
face-to-face contact hours per month)
• $450.00/Month(Therapist only, Weekly individual therapy only, Minimum 4 face-to-
face contact hours per month)
• $450.00/Month(Therapist only, Weekly family therapy only, Minimum 4 face-to-face
contact hours per month)
• $700.00/Month(Facilitator only, Minimum 6 face-to-face hours per month)
• $100.00/Hour(Partial month bulling to bused when services begins or ends mid-month
OR when minimum contact hours are not met during the month)
I do not recommend a Work Session. I recommend approval of this Renewal.

AGREEMENT EXTENSION/RENEWAL BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND POLARIS PARTNERS, LLC (CORE) AA��
This Agreement Extension/Renewal ("Renewal"), made and entered into c day of 1.4 ,2015,by and between
the Board of Weld County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter referred to as
the"Department",and Polaris Partners,LLC, hereinafter referred to as the"Contractor".
WHEREAS the parties entered into an agreement(the"Original Agreement")identified by the Weld County Clerk to the
Board of County Commissioners as Document Number 2015-0493, approved on 10106/2014.
WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the
Original Agreement,which is incorporated by reference herein,as well as the terms provided herein.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on May 31, 2015.
• The parties agree to extend the Original Agreement for an additional one(1)year period,which will begin June 1,201S,and
will end on May 31,2016.
• The Renewal,together with the Original Agreement, constitutes the entire understanding between the parties. The
following change is hereby made to the Contract Documents:
1. EXHBIT C-PAYMENT SCHEDULE, PARAGRAPH 2-FEES FOR SERVICES:
$1,700.00/Month(Therapist and Facilitator, Minimum 12 face-to-face contact hours per month)
$900.00/Month (Therapist only,Weekly individual and family therapy,Minimum 6 face-to-face contact hours per
month)
$450.00/Month (Therapist only,Weekly individual therapy only, Minimum 4 face-to-face contact hours per month)
$450.00/Month(Therapist only,Weekly family therapy only, Minimum 4 face-to-face contact hours per month)
$700.00/Month(Facilitator only,Minimum 6 face-to-face hours per month)
$100.00/Hour(Partial month billing to be used when services begins or ends mid-month OR when minimum
contact hours are not met during the month)
2. None
• All other terms and conditions of the Original Agreement remain unchanged.
