This Memorandum provides a summary of the following:
• Weld County Department of Human Services Child Protection Agreement
Extension/Renewal with Various Providers
Please review and indicate if you would like a work session prior to placing these items on the
Board's agenda.
1. Request Board Approval for the Departments'Child Protection Agreement
Extension/Renewal with Various Providers.The below parties agree to extend the
',term of their original agreement,in accordance with the conditions of the original
agreement,under the standard form for renewals.The standard form was approved as
new business on March 9,2015.The renewal term is for June I,2015 to May 31,2016

AGREEMENT EXTENSION/RENEWAL BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND GREELEY COUNSELING CENTER (CORE)
This Agreement Extension/Renewal ("Renewal"), made and entered into t day of,1 A
hC ,,2015,by and between
the Board of Weld County Commissioners,on behalf of the Weld County Department of Human Services, hereinafter referred to as
the"Department",and Greeley Counseling Center, hereinafter referred to as the"Contractor".
WHEREAS the parties entered into an agreement(the"Original Agreement") identified by the Weld County Clerk to the
Board of County Commissioners as Document Number 2014-3338,approved on 10/27/2014.
WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the
Original Agreement,which is incorporated by reference herein,as well as the terms provided herein.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on May 31,2015.
• The parties agree to extend the Original Agreement for an additional one (1)year period,which will begin June 1,2015,and
will end on May 31, 2016.
• The Renewal,together with the Original Agreement,constitutes the entire understanding between the parties. The
following change is hereby made to the Contract Documents:
1. Exhibit D, Payment Schedule, Paragraph 2-Fees for Services:
$1,526.00/Episode(Psychological Evaluation—Adult or Adolescent, In-Office)
$1.861.00/Episode(Psychological Evaluation—Adult or Adolescent,Out-of-Office)
$1,045.00/Episode(Parental Fitness Evaluation)
$192.50/Hour(Partial Psychological Evaluation)
$176.00/Hour(Interactional Evaluation)
$168.00/Hour(Crisis Intervention)
$146.00/Hour(Case Consultation)
$126.00/Hour(Individual Therapy)
$126.00/Hour(Family Therapy)
$126.00/Hour(Court Facilitation/Court Staffing/Family Team Meeting/Team Decision Making Meeting)
$62.00/Episode(Travel Surcharge)
2. None
• All other terms and conditions of the Original Agreement remain unchanged.
