RE: APPROVE STANDARD FORM FOR CONTRACT AGREEMENT AMENDMENT
BETWEEN THE DEPARTMENT OF HUMAN SERVICES AND VARIOUS PROVIDERS
AND AUTHORIZE CHAIR TO SIGN DOCUMENTS CONSISTENT WITH SAID FORM
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board, sitting as the Weld County Board of Human Services, has been
presented with the form of a Contract Agreement Amendment between the Weld County
Department of Human Services and various providers, and
WHEREAS, after review, the Board deems it advisable to approve the form of said
Contract Agreement Amendment, a copy of which is attached hereto and incorporated herein by
reference, and to delegate standing authority to the Chair of the Board of County Commissioners
to execute individual amendments between the Department of Human Services and various
providers.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, sitting as the Weld County Board of Human Services, that the form of said
Contract Agreement Amendment between the Weld County Department of Human Services and
various providers be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign any agreements consistent with said form.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of November, A.D., 2015.

Welfare Contract Agreement Amendment Template
• Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval for the Departments' Child Welfare Contract Agreement
Amendment Template(Attachment 1). This template has been reviewed and approved by
County Attorney Frank Haug and will be utilized in cases where amendments are necessary on
the following Human Services Child Welfare agreements: Child Protection Agreement for
Services,Agreement for Respite Services,Agreement for Professional Services,Agreement to
Purchase Out-of-Home Services SS23A,and Individual Provider Contract for Purpose of Foster
Care Services and Foster Care Facility Agreement CWS-7A.The Department is asking that the
template be approved and such agreements be allowed to go directly to consent agenda.
I do not recommend a Work Session. 1 recommend approval of this Agreement Amendment
Template and such agreements go directly to consent agenda.

of Weld County Commissioners,on behalf of the Weld County Department of{Dept.Name',hereinafter referred to as the
"Department",and JCONTRACTOR NAME',hereinafter referred to as the"Contractor".
WHEREAS the parties entered into an Agreement to ,(the"Original Agreement")identified by the
Weld County Clerk to the Board of County Commissioners as document No.{TYLER#1,approved on JBOCC Approval

WHEREAS the parties hereby agree to amend the term of the Original Agreement in accordance with the terms of
the Original Agreement,which is incorporated by reference herein,as well as the terms provided herein.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
• The Original Agreement will end on{Expiration Datel.
• The Amendment,together with the Original Agreement,constitutes the entire understanding between the parties.
The following change is hereby made to the Contract Documents:
