Approve Local Public Health Agency (LPHA) Application for Fiscal Years 2027-2029
State Tobacco Education and Prevention Partnership (STEPP) Grant Program, and
Authorize Chair to Sign Authorize Department of Public Health and Environment to
Submit Electronically,
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Local Public Health Agency (LPHA)
Application for Fiscal Years 2027-2029 State Tobacco Education and Prevention
Partnership (STEPP) Grant Program from the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department
of Public Health and Environment, to the Colorado Department of Public Health and
Environment, with further terms and conditions being as stated in said application, and
Whereas, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Local Public Health Agency (LPHA) Application for Fiscal
Years 2027-2029 State Tobacco Education and Prevention Partnership (STEPP) Grant
Program from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, to the Colorado Department of Public Health and Environment, be, and
hereby is, approved.
Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign
said application via the Signature Authority Form, and Cathy Hoover, Department of
Public Health and Environment, be, and hereby is, authorized to electronically submit said
application.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 1st day of December, A.D., 2025:

Brief description of the problem/issue:
The Weld County Department of Public Health & Environment (WCDPHE) receives funding for 2.5 FTE from
State Tobacco Education Prevention Program. We seek to maintain this program at the current level and the
FY 2027-2029 funding cycle requires an interest form as the initial step for continued funding. A formal contract
and budget will be brought to the Board in 2026.
What options exist for the Board?
Approve and sign, or deny this Interest Form
Consequences: Approving and signing this agreement will enable WCDPHE to continue to provide
tobacco education and cessation services at current program levels.
Impacts: Approving this interest form will initiate the next 3-year contract which will include sustaining
2.5 current FTEs.
Costs (Current Fiscal Year/Ongoing or Subsequent Fiscal Years):
Funding is anticipated to be approximately $412,000. The final amount will be set in 2026. There is no county
match required, and all program activities are fully grant funded. No county funding or additional FTE is
requested.
Recommendation:
I recommend approval to place this item on a future BOCC agenda for formal consideration.

Based on the stated STEPP 2030 Strategic Plan goals for commercial tobacco control
work in Colorado, please briefly describe the following:
Your current agency and program infrastructure and capacity to implement a
commercial tobacco control program(300 words or less).*
Our Weld County Tobacco Education and Prevention Program (ECTEPP) is located within
the Weld County Department of Public Health and Education (WCDPHE) in Greeley.We are
supported by an Interim Division Director, Department Director, and a robust fiscal team.
Cross-collaboration with other department programs and good relationships with the
individuals in them help to strengthen our team as well. Our health department is
accredited through the Public Health Accreditation Board (PHAB).
The WCTEPP is currently made up of 2.5 FTE;two full-time Tobacco Education Specialists
and one (.5 FTE) Program Manager. Since the Latino population represents roughly 30%of
Weld County residents and is rapidly growing, both Tobacco Education Specialists are
bilingual in English and Spanish, and we partner with the Community Health Division's
Community Health Workers on further outreach. Much of this work is connecting the Latinx
Community to Health resources including tobacco cessation.Additionally,we are
supported by an internal team of highly qualified data analysts who readily provide us with
data from the Colorado Quitline, BRFSS,TABS, HKCS and more importantly local data via
the Community Health Assessment(completed every 3 years).They also assist with
projects such as the STEPPS Community Profile.

staff, representative of the community's racial and cultural diversity(300 words or

Describe the scope of community support and identified or anticipated challenges for
your program and agency as the lead organization in the implementation of the

Community support:WCDPHE partners with numerous local agencies. As a tobacco
control program, we believe in authentic community engagement and meeting people
where they are. This model has helped us build relationships and trust with key leaders in
community agencies such as United Way, Luther Family Services, North Range Behavioral
Health, School Districts, Chambers of Commerce, medical and dental clinics, various
community coalitions, and businesses such as JBS, the largest employer in Weld County.
This bidirectional relationship model has also increased trust with individuals in the
community and taught us what is important to them and how to structure messaging and
programming within our community.
Challenges:Weld County prides itself on being comprised of independent thinkers and
people who don't want to be"told what to do"especially if it comes from the government.
As a Health Department, we have to proceed carefully with education and policy. We strive
to promote healthy choices framed in a way that makes sense and relates to people's
experience and lifestyle without infringing on their freedoms.
Our 2022 Weld County Community Health Survey Data showed an increase in e-cigarette
use and vaping especially among 18-34-year-olds. This will require us to pivot in our
education and messaging to reach this population and also consider the environmental
impact of e-cigarettes. We are already aware of limited capacity and system barriers within
our County Household Hazardous Waste to receive and dispose of the toxic waste caused
by e-cigarettes.
Limited data creates a disadvantage in our youth work. In 2021, only a few of our Weld
County schools participated in the Healthy Kids Colorado Survey and the Community
Heath Survey only polls individuals ages 18 and up.
Describe your agency's capacity to assess your community needs to implement
the Community Tobacco/Nicotine Initiatives Framework for Local Public Health
Agencies, including tools and strategies your agency has used to assess community
needs(300 words or less).*
As part of our Community Health Assessment process,WCDPHE's Data Team conducts a
representative, simple random-sample Community Health Survey of county residents
every three years which includes rich, detailed information on tobacco use among Weld
County adults that can be disaggregated by factors including race/ethnicity, age, gender,
sexual orientation, income, county region, and educational attainment. Having our own
high-quality, local data source and a team of data analysts enables WCDPHE's tobacco
team to assemble a more detailed assessment of tobacco-related community needs than
we would be able to use state or national data sources.To incorporate youth needs into our
assessments,we rely on Healthy Kids Colorado Survey data and have maintained a close
working relationship with HKCS administrators to improve participation by Weld County
Schools.The WCDPHE tobacco team has also leveraged the relationships we've
developed in local school systems through delivering tobacco education programming to
recruit professionals who work with youth to participate in Key Informant Interviews, in
order to add qualitative data to our assessments. In addition to the Weld Community
Health Survey, the Healthy Kids Colorado Survey, and Key Informant Interviews,we have
incorporated data from the Behavioral Risk Factor Surveillance Survey and the Weld Intake
Demographic Report for Quitline.
In order to staff this program accurately, STEPP requires that agencies have at least
a 0.75 dedicated FTE in place by the end of the first grant year. Describe the agency's
plan to hire, onboard,and retain qualified commercial tobacco control staff
representative of the community's racial and cultural diversity,that meets this
requirement.*
As stated above,there are 2.5 FTE on this project. Combined with diverse strategies to
recruit new staff,the additional bilingual pay and dedication to ongoing learning
opportunities exemplify our commitment to employee retention.
