RE: APPROVE AGREEMENT FOR INMATE MEDICAL SERVICES AND AUTHORIZE
CHAIR TO SIGN - CORRECT CARE SOLUTIONS, LLC
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for Inmate Medical
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Sheriff's Office, and Correct Care Solutions,
LLC, commencing January 1, 2017, and ending January 31, 2017, with further terms and
conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Agreement for Inmate Medical Services between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf
of the Sheriff's Office, and Correct Care Solutions, LLC, be and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of December, A.D., 2016.

CONTRACT AGREEMENT EXTENSION/RENEWAL BETWEEN
THE WELD COUNTY Sheriff's Office
AND Correct Care Solutions, LLC

This Agreement Extension/Renewal ("Renewal"), made and entered into on this day of December, 2016, by and
between the Board of Weld County Commissioners, on behalf of the Weld County Sheriff's Office, hereinafter referred to as the
"Department", and Correct Care Solutions, LLC, hereinafter referred to as the "Contractor".
WHEREAS the parties entered into an agreement (the "Original Agreement") identified by the Weld County Clerk to the
Board of County Commissioners as document No. 2012-3051, approved on 11-30-2012.
WHEREAS the parties hereby agree to extend the term of the Original Agreement in accordance with the terms of the
Original Agreement, which is incorporated by reference herein, as well as the terms provided herein.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
• The Original Agreement will end on 12-31-2016.
• The parties agree to extend the Original Agreement for an additional 1 month period, which will begin 01-01-2017, and will
end on 01-31-2017.
• The Renewal, together with the Original Agreement, constitutes the entire understanding between the parties. The
following change is hereby made to the Contract Documents:
1. The monthly amount to be paid by Weld County to the contractor under this agreement is $286,255.21 (Two Hundred
Eighty Six Thousand Two Hundred Fifty Five Dollars and Twenty One Cents) for the period beginning January 1, 2017
through January 315t, 2017.
• All other terms and conditions of the Original Agreement remain unchanged.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day, month, and year first above written.
