RE: APPROVE DESIGNATION OF THE AUTHORIZED REPRESENTATIVE, ACCOUNT
MANAGER,AND SUBMISSION AGENT FOR SECTION 111 MANDATORY REPORTING
APPLICATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, Section 111 of the Medicare, Medicaid, and SCHIP Extension Act of 2007
(MMSEA) added mandatory reporting requirements with respect to Medicare beneficiaries who
receive settlement, judgements, awards or other payments from liability insurance, or workers
compensation plans, and
WHEREAS, provisions of 42 U.S.C. 1395y(b)(8) added reporting requirements for
self-insured worker compensation plans, such as Weld County's, as responsible reporting entities,
and
WHEREAS, the Section 111 mandatory reporting requirements require the self- insured
worker compensation plans to designate an Authorized Representative who has ultimate
accountability for the responsible reporting entity's compliance with Section 111, an Account
Manager, and a Submission Agent, and
WHEREAS, the Director of Finance and Administration has reviewed the Section 111
mandatory reporting requirements and recommends that Patti Russell, Director of Human
Resources, be designated as the Authorized Representative, Michelle Raimer, Risk Manager, be
designated as the Account Manager, and the third party administrator of the workers'compensation
plan, CTSI, be designated as the Submission Agent for the Weld County Self-Insured Workers
Compensation Program, and
WHEREAS, the Board of Weld County Commissioners deems it advisable that for the
Section 111 mandatory reporting requirements Patti Russell be designated as the Authorized
Representative, Michelle Raimer be designated as the Account Manager, and the third party
administrator of the workers' compensation plan, CTSI, be designated as the Submission Agent for
the Weld County Self-Insured Workers Compensation Program.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Patti Russell be designated as the Authorized Representative, Michelle
Raimer be designated as the Account Manager, and the third party administrator of the workers'
compensation plan, CTSI, be designated as the Submission Agent for the Weld County Self-Insured
Workers Compensation Program effective immediately.

Subject: Section 111 Mandatory Reporting
Section 111 of the Medicare, Medicaid, and SCHIP Extension Act of 2007 (MMSEA) added mandatory
reporting requirements with respect to Medicare beneficiaries who receive settlement, judgements,
awards or other payments from liability insurance,or workers compensation plans. Provisions of 42 U.S.C.
1395y(b)(8) added reporting requirements for self-insured worker compensation plans, such as Weld
County's, as responsible reporting entities.
The Section 111 mandatory reporting requirements require the self-insured worker compensation plans
to designate an Authorized Representative who has ultimate accountability for the responsible reporting
entity's compliance with Section 111, an Account Manager, and a Submission Agent. I have reviewed the
Section 111 mandatory reporting requirements and recommend that Patti Russell, Director of Human

designated as the Account Manager,and the third party administrator of the workers'compensation plan,
CTSI, be designated as the Submission Agent for the Weld County Self-Insured Workers Compensation
Program.
