RE: APPROVE CORRECTION TO ONE (1) OIL AND GAS LEASE TERM AND AUTHORIZE
CHAIR TO SIGN - EXTRACTION OIL AND GAS, LLC
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, on April 7, 2015, the Board approved a Lease with Synergy Resources
Corporation, 1625 Broadway, Suite 300, Denver, CO 80202, recorded on May 21, 2016,
Reception Number 4109060, and
WHEREAS, the Board has consented to the Assignment from Synergy Resources
Corporation to Extraction Oil and Gas, LLC, effective November 13, 2015, and
WHEREAS, the Board has been presented with a Correction to said Oil and Gas Lease
term date to be April 7, 2018, between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, and Extraction Oil and Gas, LLC, with terms
and conditions being as stated in said correction, on the following described mineral acres:
W1/2 SW1/4 of Section 11, Township 5,
Range 66 West of the 6th P.M., Weld
County, Colorado
WHEREAS, after review, the Board deems it advisable to approve said correction of lease
term date to be April 7, 2018, a copy of request which is attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Correction to said Oil and Gas Lease term date between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
and Extraction Oil and Gas, LLC, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said correction.

Correction of Oil and Gas Lease Term
WHEREAS, the undersigned, WELD COUNTY, COLORADO, a political subdivision of the
STATE OF COLORADO, acting by and through the BOARD OF COUNTY COMMISSIONERS OF THE
COUNTY OF WELD, as lessor, did under date of April 7, 2015, execute unto Synergy Resources
Corporation certain Weld County Small -Tract Oil and Gas Lease at Weld County Reception
number 419060, more particularly described as:
Township 5 North, Range 66 West, 6th P.M.
Section 11: All that part of the W/2SW/4, more particularly described in that Quit -Claim
Deed to Weld County, State of Colorado, recorded December 12, 1921, under Reception
No. 368932, County of Weld, State of Colorado.
And WHEREAS, the lease term in the Lease referenced in the above description is
incorrectly stated and is inaccurate, it is the intent of the undersigned to correct the language in
the referenced Lease to read as follows:
"TO HAVE AND TO HOLD said land, and all right and privileges granted hereunder to
Lessee until the hour of twelve o'clock noon on the 7th day of April, 2018, as primary term, and
so long thereafter as oil and gas, or either of them is produced in paying quantities from said land
or Lessee is diligently engaged in bona fide drilling or reworking operations on said land, subject
to the terms and conditions therein"
NOW, THEREFORE, in consideration of the premises, and for the purpose of making said
leases more definite and certain in respect to the length of the primary term intended to be
covered thereby, the undersigned does hereby declare that it was and is the intention of the
undersigned to amend said Lease referenced in the above description in respect to the length of
the primary term, as aforesaid, and to ratify and adopt the same as so amended.
