RESOLUTION
RE: DENTAL SERVICES CONTRACT BY THE SALUD DEL VALLEY
CLINIC.
WHEREAS, the Salud DelValley Clinic, a non-profit Colorado
Corporation, has submitted a contract to the Board of County Commis-
sioners, Weld County, acting as the Weld County CommunityAction
Agency for a term beginning October 21, 1974, and ending May 31, 1975,
a copy of which is attached hereto and made a part hereof by reference,
and
WHEREAS, after careful review and consideration of the con-
tract, the Board believes it to be in the best interest of the County to
accept the same.
NOW, THEREFORE, BE IT RESOLVED, by the Board of County
Commissioners, Weld County, Colorado, that the Dental Services Contract
as submitted by Salud Del Valley Clinic be, and the same is hereby ap-
proved and accepted for signature by the Board.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 2nd day of
April , A.D., 1975, nunc pro tune as of October 21st , 1974.

AGREEMENT
THIS AGREEMENT, is made and entered into at Greeley,
Colorado, this 21st day of October, 1974, by and between WELD COUNTY,
COLORADO, acting as the Community Action Agency, hereinafter referred
to as THE AGENCY and the SALUD DEL VALLEY CLINIC, hereinafter re-
ferred to as THE CONTRACTOR, both of Weld County, Colorado.
WITNESSETH:
That THE AGENCY and THE CONTRACTOR do mutually agree
as follows:
1. THE CONTRACTOR shall, in a satisfactory and proper man-
ner as determined by THE AGENCY, perform the following:
Dental Care services to include:
1. Dental prophylaxis and instruction in self care and
hygiene procedures.
2. Application of tropical fluoride.
3. Restoration of decayed primary and permanent
teeth.
4. Pulp therapy for primary and permanent teeth.
5. Extraction of non-restorable teeth.
6. Services required for the relief of pain or in-
fection.
FOR:
Forty-five (45) children x Twenty-five dollars ($25. 00) =
One Thousand One Hundred and Twenty-five dollars ($1125. 00).
2. THE CONTRACTOR shall commence performance on this
Contract on the 21st day of October, 1974, and shall complete performance
to the satisfaction of THE AGENCY no later than the 31st day of May, 1975.
3. THE CONTRACTOR shall maintain such records and accounts,
including property, personnel and financial records as are deemed necessary
to THE AGENCY or the Director of HEW to assure a proper accounting for
all project funds, both Federal and Non Federal shares. These records will
be made available for audit purposes to THE AGENCY, the HEW, or
Comptroller General of the United States or any authorized representative,
and will be retained for three years after the expiration of this contract
unless permission to destroy them is granted by both THE AGENCY and
the Director of HEW.
4. Services provided for under this Agreement shall be comp-
ensated for by THE AGENCY in the following manner:
Payments for services provided will be made no later than
December 31, 1974.
5. It is expressly understood and agreed that in no event will the
total amount to be paid by THE AGENCY to THE CONTRACTOR under this
Agreement will exceed One Thousand One Hundred and Twenty-five dollars
($1125. 00), for full and complete satisfactory performance. THE CON-
TRACTOR will not be obligated to render services in excess of this amount.
6. This Agreement is subject to and incorporated the attached
Part II, "Terms and Conditions Governing Contracts Between Community
Acting Program Agency and Contractor for Professional or Technical Ser-
vices to a Community Action Program. "
7. THE CONTRACTOR agrees to assist THE AGENCY in complying
with all the "Conditions Governing Grants under Sections 204 and 205 of the
Economic Opportunity Act of 1964. "
8. IN WITNESS WHEREOF, THE AGENCY and THE CONTRACTOR
have executed this Agreement as of the date first above written:
