Wallace, MD, MPH Executive Director Department of Public Health & Environment DATE: March 17, 2016 SUBJECT: Colorado Mosquito Control Contract Agreement Extension/Renewal Enclosed for the Board's review and approval is the Contract Agreement Extension/Renewal with Colorado Mosquito Control (CMC), an email from the contractor agreeing to extend the original agreement without increasing prices for 2016, and a copy of the 2014 bid/contract agreement (resolution number 2014-0672, approved on March 19, 2014). On March 19, 2014, the County entered into an agreement with CMC to conduct mosquito surveillance service in Weld County. The agreement was extended with no pricing increase for 2015, and CMC has again agreed to extend the agreement once more for 2016, with no price increase from the original 2014 bid/contract. This extension would be for the third and final year of a possible three-year contract term limit. The proposed costs of $14,736.80, or $1,133.60 per week for 13 weeks, for the time period of June 1, 2016, through August 31, 2016, are in line with the original bid/contract and accounted for in the 2016 budget. This includes 15 sentinel zone traps and one additional historical surveillance trap (located in Fort Lupton) per week. The Department has been satisfied with the services provided by Colorado Mosquito Control. In spring 2014, the County entered into an agreement with CMC to conduct mosquito surveillance service in Weld County. The original agreement for mosquito surveillance can be extended from May 2015, to December 2015, as permitted by the bid/contract. CMC has agreed to extend the 2014 pricing with no increases for 2015. This extension would be for the second year of a possible three year contract term limit.
