1) OPTIONS:
a. Extend for an additional term.
2) REQUIRED PROVISIONS:
In accordance with Section 5, TERM, Subsection C, Option to Extend, of the Original Contract between the
State of Colorado, Department of Health Care Policy and Financing, and Weld County Department of Human
Services, the State hereby exercises its option for an additional term beginning on July 1, 2016 and ending on
June 30, 2017.
To account for the extension of the term of the Contract, the Maximum payable under the Contract for the
Contractor is decreased to $1,690,395.00, the State-wide Quality of Life Interviews maximum is increased to
$37,100.00 and the following changes are hereby made to the Contract:
The Contractor Maximum table in Section 7, PAYMENTS TO CONTRACTOR, Subsection A, Maximum
Amount is hereby deleted in its entirety and replaced with the following:

ALL CONTRACTS REQUIRE APPROVAL BY THE STATE CONTROLLER
CRS §24-30-202 requires the State Controller to approve all State Contracts. This Contracts is not valid until signed and dated below
by the State Controller or delegate. Contractor is not authorized to begin performance until such time. If Contractor begins
performing prior thereto, the State of Colorado is not obligated to pay Contractor for such performance or for any goods and/or
services provided hereunder.
