WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Promoting Safe and Stable Families
(PSSF) Partnership Project Application from the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Division of Child Welfare, in Collaboration with Catholic Charites and
Community Services of the Archdiocese Denver Weld Regional Office, to the Colorado Office of
Early Childhood, with further terms and conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.

Enclosed for review and consideration by the Colorado Office of Early Childhood is our
application for Promoting Safe and Stable Families (PSSF) funding. This application shall
remain valid for a minimum of one hundred eighty (180) days from the date of the award. The
Weld County Department of Human Services (WCDIIS), Division of Child Welfare, a
government entity acting as fiscal agent, is the Offeror. The Offeror is submitting this application
in collaboration with Catholic Charities and the Community Services of the Archdiocese of
Denver (Catholic Charities), a charitable organization. We are willing to comply with all work
requirements and other terms and conditions as specified in this Request for Application (RFA).
In addition to this transmittal letter, the enclosed application is comprised of the following
materials and attachments:

Neither WCDHS nor Catholic Charities, nor any of its employees, agents, independent
contractors, or proposed subcontractors has ever been an excluded provider, convicted of any
criminal charges or actions involving moral turpitude. Weld County Department of Human
Services does not have any current pending contracts with the State of Colorado; however,
Catholic Charities has three: 1I7ESG16915, II7ESG16960 and H7ESG16952. Neither WCDHS
nor Catholic Charities has any bids or applications submitted to the State of Colorado but not yet
awarded. Furthermore, neither entity has identified any potential conflicts of interest related to
the RFA. Neither the vendor (WCDHS) nor the subcontractor (Catholic Charities) will perform
the services outlined in this RFA outside the state of Colorado or the United States.

COMPONENT B
TECHNICAL/BUSINESS APPLICATION
AGENCY EXPERTISE AND EXPERIENCE
Introduce the provider agency and geographic region included in the proposed site
collaboration.
Weld County Department of Human Services (Weld County DHS) is located in northern Colorado
and is home to 31 incorporated municipalities that include growing cities and acres of prime
agricultural land. Weld is the third largest county in Colorado, covering 3,987 square miles with a
population of approximately 269,785. For the past several years, Catholic Charities and
Community Services of the Archdiocese of Denver has been a Promoting Safe and Stable Families
(PSSF) grantee and then entered into an agreement with Weld County DHS to be its partner in
implementing the grant. This year, with the stipulation that grant funding can be awarded to
counties only, Weld County DHS is submitting the application with the intent to continue its
partnership with Catholic Charities and is applying for a funding award level of $120,000.
Describe the provider agency's service array and the populations(s) currently being served.
Be sure to include any experience delivering PSSF services, experience working with family,
children, and experience delivering voluntary services.
Weld County DHS collaborates with Catholic Charities and Community Services of the
Archdiocese of Denver to provide PSSF services. Catholic Charities' offices in Weld County are
housed at Guadalupe Community Center, which is Catholic Charities' homeless shelter and
administrative office. Catholic Charities has been providing PSSF services to at -risk and kindship
families since 1999, strengthening its relationship with Weld County DHS during that time relative
to the collaborative provision of services to families. During the first 12 years of the PSSF program,
Catholic Charites worked with families in each of the PSSF service categories. More recently,
Catholic Charities and Weld County DHS developed a new collaborative model: the Weld County
Promoting Safe and Stable Families Partnership. Through this model, Catholic Charities focused
on providing Adoption/Kinship support and Family Preservation services to families staying at
Guadalupe Community Center. Weld County DHS focused on providing Family Support Services
to families residing either at the shelter or in the community who could benefit from this early
intervention service. Communication and collaboration by Catholic Charities and Weld County
DHS strengthened service delivery. Some services were provided jointly when it was beneficial to
the family. These included parent training and support groups with either Catholic Charities or
Weld County DHS taking the lead.
Provide a community profile that describes the impact of risk factors as they relate to parents
and caregivers in the proposed target geographic service area. Explain how your agency
assessed the current needs of your region (i.e. collaborative needs assessment process with
other agencies and parents, focus groups, accessing other agency's data/reports, etc.)

Weld County DHS participates with multiple community organizations and various taskforces,
enabling it to remain current on family crisis needs that are observed by these service entities.
Weld County DHS and Catholic Charities have participated in a yearlong community assessment
and strategic planning taskforce, known at the Homeless Coalition. The goal of the Homeless
Coalition is to construct a multi -faceted community plan — "Weld Way Home" — to reduce the
growing homeless population in Weld County. For many, many years, Catholic Charities has
served as the safety net for homeless families through Guadalupe Community Center. Weld
County DHS, Catholic Charities and Greeley Transitional House have developed a very efficient
and functional communication and emergency planning collaboration to serve the emergency
needs of homeless families.
Provide evidence that PSSF services being applied for in your community does not duplicate
other services offered in your community. If other programs already exist, describe how the
PSSF funding is still a needed service that will be utilized in the community how it will be
integrated with other community based programs, policies, practices, and approaches
addressing the same or similar need. Applicants should reference how the proposed program
fits into the community continuum of services, if it fills an identified gap in services, and/or
works to eliminate barriers to a family's ability to access services.
Catholic Charites provides emergency assessments of, and planning for, homeless families and
approves short-term hotel vouchers that are funded by Weld County United Way. Weld County
DHS may also provide emergency assessments and hotel vouchers through Community Service
Block Grants, while Greeley Transitional House provides shelter for families when it has space
available. Weld County DHS, Catholic Charities, and Greeley Transitional House communicate
with one another and share resources in order to provide comprehensive emergency services for
families. Between 12 and 15 families per month are referred for emergency screening and safety
planning to this emergency response team. In Weld County, there is a significant shortage of
shelter rooms for families and long waiting lists. Because of this, a family may utilize many entry
points into receiving the help they need.
Provide a description of prior experience providing related services. Describe how previous
experience provides evidence of the capacity to successfully achieve the expected outcomes.
Please describe reason or expertise in working with the specific service categories for which
you are applying.
Because one of the new focuses of service delivery is on kinship families, there is an increased
need for supportive services in order to ensure a stable family environment. This includes concrete
services such as rent, utility assistance, transportation assistance, respite care and other hard goods
that families will require to sustain permanency.
Weld County DHS has a strong focus on father involvement. In an effort to support fathers, the
agency continues to develop and create a fatherhood program that can be branched out to local
community partners such as Catholic Charities, the City of Greeley, Community Corrections, etc.
Since approximately 2007, Weld County DHS has had an initiative to work with various grants
and organizations to develop and support a fatherhood program. The fatherhood program currently
utilizes the Circle of Parents model which focuses on creating a place where anyone in a parenting

role can openly discuss the successes and challenges of raising children in a supportive parent -led
group. Weld County DHS has worked collaboratively with Catholic Charities to support kinship
families, which are increasing in number. Because of this, there is a need for expanded services to
support these placements. In 2016, the Weld County Department of Child Welfare had
approximately 333 children placed in kinship out -of -home placements. The goal is to establish a
support group for families and children/youth that will educate, support and reinforce the
importance of permanency for children/youth.
Describe the organizational structure and how the organization will manage this project. If
applicable, indicate key personnel who will be assigned to the program and describe the
experience of each. Include any relevant education and/or certifications that organization
employees have obtained. Indicate how these staff will be supervised.
The contractor for the PSSF program will be Weld County DHS, which will coordinate services
with Catholic Charities as a subcontractor. At Weld County DHS, Geraldine Romero is currently
the Community Case Manager. Geraldine has a bachelor's degree and has worked for 17 years in
the child welfare field. Geraldine had strong skills in parent education with a focus on permanency,
trauma and child development. Toi Lynn Edwards is the Kinship Supervisor. She has a bachelor's
degree and over 20 years' experience in child welfare at both child placement agencies and public
agencies. She has extensive experience in foster care and kinship services. At Catholic Charities,
Enita Kearns-Hout, Weld County Regional Director, has a bachelor's degree and 30 years of direct
practice with foster and adoptive families and children with DHS agencies. She also worked 12
years as a permanency specialist trainer for the Adoption Exchange. Cheri Anderson is Catholic
Charities' Director of Case Management. She has a Master of Social Work degree and has worked
in family and children's services for 20 years. As supervisor of the PSSF program, she has gained
extensive experience in working with at -risk families. Kraig Patriquin provides Intensive Case
Management to families staying at Guadalupe Community Center. He has a Master of Social Work
degree and has extensive training in issues related to mental health, motivational interviewing and
problem -solving therapy. Sandra Abagail Garcia has a Bachelor of Science degree in psychology
and will support monolingual families at Guadalupe Community Center and at the new permanent
supportive housing apartments (which open in May) adjacent to the shelter. Catholic Charities will
be hiring an additional staff member to serve the at -risk families at the new apartments.
How will the provider agency retain PSSF staff, and ensure there is no gap in service delivery
if the agency experiences staff turnover?
Weld County DHS has an established Kinship Unit. Within the unit, the agency retains a Kinship
Supervisor and three Kinship Coordinators who have the expertise to provide supportive services
in the event of staff turnover or absence. Catholic Charities will have a total of 3.5 FTE social
work practitioners serving clients at Guadalupe Community Center and at the permanent
supportive housing apartments. This will allow for continuation of services if the agency
experiences turnover. Catholic Charities' Director of Case Management also has the expertise to
provide direct services in the event of staff turnover or absence.
Provide an explanation of the organization's commitment to be responsible to a family's
cultural considerations.

Weld County DHS will have one Spanish-speaking case manager to serve monolingual families.
The agency's Community Case Manager and Kinship Coordinator are bilingual and bicultural. All
agency staff members are culturally aware and receive ongoing cultural training. Catholic Charites
has one bilingual and bicultural case manager dedicated to working with families at Guadalupe
Community Center. A second bilingual case manager will be hired when the permanent supportive
housing apartments open. Cultural competency and the understanding that families have diverse
histories and cultures and values is a core training expectation for Catholic Charities staff
COMMUNITY PARTNERSHIPS AND COLLABORATIONS
Describe collaborative relationships with a minimum of four key partners in the proposed
geographic community and how they will contribute to the PSSF program's success,
including any other post -adoption programs.
Weld County DHS and Catholic Charities have a strong history of collaboration relating to Child
Welfare Services/Promoting Safe and stable Families. Guadalupe Community Center is a one -stop
service delivery center, providing programs that help all ages — from infants to seniors. Within this
continuum of care there is a focus on homeless prevention/emergency services, transitional shelter
programs and the aforementioned supportive housing apartments that will include 10 units for
families. Other partners that provide expanded and wraparound services include:
North Range Behavioral Health: Supports the mental health needs of families with on -site
individual assessments, support groups and immediate referrals for individual treatment and
medication assessments. The agency has a 12 -bed crisis facility and mobile crisis unit that responds
to mental health needs 24 hours a day, seven days a week.
Sunrise Community Health Care Services: This agency's mobile health van comes to
Guadalupe Community Center twice a month. The partnership facilitates same -day office visits
when the need for immediate medical attention is indicated.
Cheyenne VA Medical Center: Medical Center staff visit Guadalupe Community Center weekly,
providing case management to veterans.
Volunteers of America: Provides veterans at Guadalupe Community Center with services.
High Plains Library: Provides technology and computer training at Guadalupe Computer Center.
Weekly classes support development or enhancement of computer skills, resume writing and
carrying out a job search. The library also provides the shelter's Children Always Learning Lab
with
iPads, learning apps and books.
Describe new opportunities and plans to develop and expand partners in the proposed
community.

For Weld County DHS, building relationships with cities and service providers is critical to
providing services to Weld County residents. The department reaches out annually to many
agencies in order to contract with highly qualified CORE services providers for families and
children.
Guadalupe Community Center and the new permanent supportive housing apartments are on a 12 -
acre site. With office space and rooms that can be utilized by various groups, the opportunity exists
to bring in more service providers to meet client needs. The new apartments will have on -site case
managers from Catholic Charites and a mental health specialist from North Range Behavioral
Health. Community meetings held before the opening of the new apartments will open the door to
communication and awareness with an opportunity to expand partnerships.
If sub -contracting with a community based organization, provide a memorandum of
understanding (if available). Explain how the county will ensure program services are
delivered as intended.
Memorandum of Understanding will be completed with Catholic Charities upon notification of
award.
PROGRAM DESIGN
Describe the services to be provided and the target population for the services.
The target population is families with children at -risk of abuse, neglect and out -of -home
placements. Homeless parents and children suffer serious ill effects that can have long-term
physical and mental health consequences. Homeless women often report being physically and
sexually abused, and children in homeless families may have been exposed to violent events and
trauma. Homeless children live with uncertainty in their lives, often suffer from frequent moves of
homes and schools and are at risk of abrupt moves and separations from parents. The frequent
moves and family instability have negative consequences for academic achievement and the
child's psych -social development and physical health. An overreaching focus for this collaborative
is family economic and housing stability. Services provided by the two agencies will be strength -
based, motivational and culturally appropriated and supportive.
Weld County DHS, through collaboration with Catholic Charites, will provide Family Support
Services. Families will receive Intensive Case Management and parent skill development. This
development is taught through the Parenting Wisely, Circle of Parent, Magic 1-2-3, and Love and
Logic trainings and through the Fatherhood Program. Families at Guadalupe Community Center
and the adjacent permanent supportive housing apartments will receive 8 to 12 hours per week of
consultation and direct services from Weld County DHS. The direct services provided include
additional case management services, referrals and family trainings, which may be co -facilitated
with Catholic Charities. In addition, Weld County DHS and Catholic Charities will facilitate a
monthly educational/support group for kinship families and children. Again, collaboratively, Weld
County DHS and Catholic Charities will provide Family Preservation Services with Catholic
Charities providing intensive case management to Guadalupe Community Center residents and
residents of the supportive housing apartments. These families will have access to the parent

strengthening training models provided by Weld County DHS. All of the programs provided target
families that are at risk of entry or re-entry into multiple systems.
REQUESTED DATA:
1. Population estimates ages 0-17: According to the United States Census, Weld County
Quick Facts, the total estimated population in July 2015 was 285,174.
2. Poverty estimates ages 0-17 of the county being served for this application: 11.3%
3. Number of child abuse and neglect referrals reported for your county or region for
the past year: Weld County had 6,564 called -in referrals in 2016.
4. Number of open cases unduplicated children served in child welfare: In Weld County,
there were 927 open cases. This number includes adoption cases which tend to run in the
500s.
5. Number of children in out of home placement with a goal of return home: 325
6. Number of children in out of home placement legally free for adoption in the past
year: Weld County had 27 legally free children in 2016.
7. Number of completed adoption in past year: Weld County had 17 completed adoptions
in 2016.
Provide the unduplicated number the applicant is proposing to serve within each PSSF
service or activity. Describe in detail how the applicant identified this number and provide
supporting data when relevant.
In 2016, Weld County DHS received 6,564 Community Child Protection referrals. Of these, 4,173
were not recommended for further assessment.
A Weld County DHS community case manager will provide up to 16 families between Guadalupe
Community Center and the new permanent supportive housing apartments with consultation and
direct services support. In addition, the community case manager will provide approximately 190
families in the community with prevention services that help prevent abuse, neglect and out -of -
home placement. Weld County DHS currently has 187 identified kinship families within Child
Welfare who could access and/or benefit from Family Support Services. A newly offered
Education and Support Group co -facilitated by Weld County DHS and Catholic Charities will be
held monthly at Guadalupe Community Center and/or the new apartments for up to 10 families
and their children.
Catholic Charities will provide intensive case management, referrals and support services to:
• Six families at a time at the Guadalupe Community Center, with a total of 25 to 30 families
per year.
• Five to 10 non -custodial single parents residing at the Guadalupe Community Center per
year. They may participate in parenting classes that offer an opportunity to strengthen
parental relationships with children living with another parent outside of the shelter or with
grandparents/kin.
• 10 families residing at the permanent supportive housing apartments per year. These
families will also have access to Family Support Services.

• Five to 10 (out of 37) non -custodial single parents residing at the permanent supportive
housing apartments per year. They may participate in Parenting Classes that offer an
opportunity to strengthen parental relationships with children living with another parent
outside the shelter or with grandparents/kin.
If the application is a multi -county or statewide application, how will you ensure services are
accessible to all counties in the proposed region?

Describe in detail how the applicant will work with diverse populations within the
community and how representatives from various cultures will be represented within the
program.
Both agencies have Spanish-speaking staff. If a family needs services delivered in another
language, staff members would use the Language Line or a translator provided by the Global
Refugee Center in Greeley.
Describe how you will address geographic, language, and limited resources (transportation)
barriers to providing services.
Families residing at either Guadalupe Community Center or the supportive housing apartments
can easily access public transportation. There is a bus stop right across the street, and the City of
Greeley provides bus passes for residents at no cost. However, this does not mean that
transportation isn't always a challenge, especially for residents who need to get somewhere off -
hours. (The bus pickups start at 7 a.m. and end at 7 p.m.) For school children (in compliance with
the McKinney-Vento Homeless Assistance Act), District Six provides bus services to and from
Guadalupe Community Center and the child's school.
Describe how you will strengthen protective factors with the families you serve.
Intensive case management provided by both agencies has as its core for PSSF Services a focus
on recognized protective factors to stabilize families and to support families so they can meet their
children's' needs. These protective factors relate to parental resilience, social connections and
family support, parenting skills and child development. The case management must strengthen the
social and emotional health of the parents and children as well as support families through crises.
Isolation from family members and broken or unsupportive family relationship are often seen in
families experiencing homelessness. Families generally have a deep desire to mend family
relationships but need guidance and support in developing a plan for doing so. Through intensive
case management, in one-on-one and group settings, opportunities will be explored that include
awareness -building regarding family systems and communication. For a family to succeed, support
is critical. Where there is a lack of family support and/or no opportunity for repair, families will
be encouraged to grow new social relationships.
Parenting skills and knowledge of child development and age -appropriate expectations can be
taught through parent educations classes such a Nurturing Parenting. Because Guadalupe

Community Center is where the case manager works (and that, of course, is where the family
lives), the case manager has the opportunity to model constructive discipline approaches. (This
will also be true of the permanent supportive housing apartments.) Because case managers and
social workers are working with families where they live, they are afforded the time and
opportunity to work with them on child behavioral challenges in a way that isn't possible in a non-
residential setting. (An example would be modeling supportive guidance of children during meals.)
Helping families to learn to engage in their children's schooling and to contribute to their
children's growth, development and academic success can be facilitated at the shelter and the
apartments. On the campus, four nights a week, children participate in the Children Always
Learning Lab. The goal of the lab is to help children overcome gaps in learning they may have
experienced because they have had to frequently change schools. There is a strong emphasis on
teaching parents that they are their child's first and most important teacher. Families come to either
Guadalupe Community Center or the supportive housing apartments with a history of challenges.
One goal is to have them learn how to navigate community support with the guidance of their case
manager.
Describe your process of follow-up with each family served. Include method, frequency, and
documentation of information.
Families served in the community by Weld County DHS community case manager will be reached
within a week after of receiving a referral. The duration and frequency contact varies based on the
family's goals, challenges and the length of involvement needed to address family issues and its
need for support.
Families served in Intensive Case Management by Catholic Charities staff at Guadalupe
Community Center are seen by a case manager within 48 hours of entering the shelter. For families
who enter the apartments, the goal will be for them to begin their relationship with their case
manager even before they move in so that necessary support can be provided during the transition
process. The frequency of case management for families is weighted heavily in the beginning to
immediately address challenges identified and assure best success within a shelter/permanent
housing plan. Case management then takes place weekly unless a crisis dictates it be more often.
Case plans and progress notes are entered into a database called Efforts to Outcomes, which is
capable of tracking many statistics including demographics, assessments and points of service. A
family's challenges, progress and goals are also documented through CFSA 2.0 and the Arizona
Self -Sufficiency Matrix. Paper case file records are kept on family assessment tools, appointments
and program participation dates and progress.
Describe how you will inform the community of the services available through PSSF.
Weld County DHS Protection Unit staff members conduct community outreach in order to educate
the public about available resources. They reach out to schools and medical clinics and have a
presence at various local events. Brochures are also placed at key community sites. Because Weld
County DHS and Catholic Charities have longstanding relationships with many other community
service providers, these providers can be a resource for dispersing information about the PSSF
program to the public.

Describe how the program will provide outreach and engagement of families.
Family engagement takes place through two Weld County DHS agency advisory boards: Weld
Faith Council and Family and Youth Commission. Currently, families have input in improving
programs through weekly Guadalupe Community Center resident meetings, and residents of the
new apartments will have this same type of meeting on a monthly basis.
Referral process and criteria, including any Memoranda of Agreement or internal policies
needed if providing services in collaboration with another entity.
The process begins with a referral from a community partner, individual or from within Weld
County DHS. For families staying at either the shelter or apartments, the referral most generally
would come from Catholic Charites. Service criteria includes that there is a concern related to child
safety, well-being and permanency of placement. Each case is screened/evaluated to determine if
further investigation is warranted by Weld County DHS or if a referral for services to a community
caseworker or other agency is needed.
If providing case management, how you will assure there is family directed goal setting.
Family strength -based assessments through the assessment tools of CFSA 2.0 used by both
agencies serve as the vehicle for a family to examine their strengths and challenges, leading to the
design of their desired goals.
Describe in detail any flex funding you propose using for concrete supports, including how
these funds will impact the family and what additional services around those funds will be
offered to work toward self-sufficiency.
None will be proposed.
Describe any other enhancers and/or barriers you anticipate in implementing a PSSF
program in your service area and how they will be addressed.
Because the supportive housing apartments are new, of course, this will be the first year that the
PSSF project will incorporate families in this setting. This enhancement is exciting, but both Weld
County DHS and Catholic Charities realize that challenges will come along with this growth.
However, both agencies are confident that any challenges can be overcome because of the strong
working relationship that exists between the two entities.
OUTCOMES AND PERFORMANCE MEASURES

For all of the services delivered to families, the intended outcome is to strengthen and stabilize
them and prevent abuse, neglect and out -of -home placement.
Indicate the capacity and willingness of your organization to participate in data collection

responsible for the activity.
Weld County DHS and Catholic Charities have the capacity to participate in data collection and
also utilize existing systems to track needed data and generate reports.
Describe experience your agency has with maintaining information on program activities in
a data management system, including experience in assuring data integrity.
Both agencies have a long-standing history of accurately collecting data related to program
activities. Each agency has the staff and IT capability to ensure the integrity of data.
Explain how the applicant will ensure model fidelity within the selected evidence based or
promising practices.
Both agencies have certification and training in the Evidences Based Models and tools that will be
used. Experienced supervisors provide oversight to ensure model fidelity.
How will you engage in a continuous quality improvement process and who will be involved
in this? Describe how parents will be involved in the process.
Within Weld County DHS, continuous quality improvement involves management and
administrative teams that evaluate programs and look for way to improve programming on a
rotating basis. In addition, parent satisfaction surveys provide insight and guide program changes.
Catholic Charities uses an internal Performance and Quality Improvement process. This process
starts at the program level. Program directors collect information and feedback from clients and
staff about the quality and delivery of program services. This information comes from such things
as client files, program statistical information, client surveys, follow-up client phone calls and
direct feedback from clients and employees. This information is then compiled by the Performance
Management and Evaluation Coordinator, who formulates monthly reports for the CEO. At weekly
meetings, the CEO and senior management determine if any program adjustments are needed. The
agency also has a standing Performance and Quality Improvement Committee which is comprised
of a cross section of staff members. Residents of Guadalupe Community Center provide feedback
through a Resident Action Form and exit survey.
If providing family support or family preservation services, describe plans to use the CFSA
2.0 with families meeting the criteria.
CFSA 2.0 is the assessment tool used by Weld County DHS and Catholic Charities for PSSF
families.

Other assessment tools used include Your Money Your Goals (a financial assessment tool), a home
safety checklist, a Protective Factors Survey, an initial Psycho -Social Assessment and Intake
Form, the Arizona Self -Sufficiency Matrix and, when indicated, a Family Eco Map.
Describe how the agency does follow-up work with families after completion of services to
capture feedback and impact.
Within Weld County DHS, families whose cases have been closed/completed are encouraged to
ask for support in the future as needed. Catholic Charites has an Alumni Program for families who
request ongoing support on a regular basis after transitioning out of the shelter. All families
transitioning out of the shelter know they have access to crisis support and services from their case
manager and Catholic Charities' Emergency Assistance Program.
SUSTAINABILITY
Describe how the PSSF local program will continue if funding is reduced or discontinued for
this program.
Reduced or discontinued funding would cause a hardship for agencies trying to provide robust
services related to PSSF needs. There would need to be an aggressive search for grants to support
these critical services. Of concern is the awareness that grants to provide services to families and
children are limited and very competitive.
Explain agency plan to continue to provide services in the event of staff turnover.
Weld County DHS has very low staff turnover, and it is most likely that if there were turnover,
existing staff would be quickly re -assigned to fill any vacancy. Catholic Charities' Director of
Case Management has extensive experience, not only with this program but also in family
counseling and casework. Program services would continue if there were staff turnover.
Provide evidence the proposed community considers PSSF services as a priority and that the
services will be utilized.
The Weld County community has shown it values the PSSF program by its continued referral
stream, both to Weld County DHS and Catholic Charities. The community also demonstrated that
protecting homeless families and children is a priority by donating close to $1 million over a nine -
month period to support the construction of Guadalupe Community Center.
Describe the agency's plan for family recruitment and retention for involvement in PSSF
services/activities. What activities will be implemented to increase parent engagement?
Families residing at Guadalupe Community Center will receive information regarding services as
part of their orientation. Retention in the program comes in great part because of the skills,
enthusiasm and encouragement of case managers from both agencies who acknowledge small

successes and build parents' confidence that they can succeed. By partnering program participants
with a "buddy parent" (at the both the shelter and the apartments) participants have a built-in
support system. This system will also be encouraged in kinship parent education support groups.
In addition, social activity rewards and opportunities for parent breaks can contribute to retention.
Examples of incentives that encourage retention include small tokens for participation, certificates,
movie and dinner tickets and parent nights out or pampering events. Every effort will be made to
ask the community to provide these types of incentives to the program free or at a reduced cost.
