RE: APPROVE CARE COORDINATION AND COMMUNICATION AGREEMENT AND
AUTHORIZE DIRECTOR OF HUMAN SERVICES TO SIGN - NORTH RANGE
BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Care Coordination and Communication
Agreement between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and North
Range Behavioral Health, commencing upon full execution of signatures and ending with terms
and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Care Coordination and Communication Agreement between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and North Range Behavioral Health be
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Director of Human Services be, and
hereby is, authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 16th day of January, A.D., 2017.

Weld County Department of Human Services' Care and
Coordination Agreement with North Range Behavioral
Health (NRBII)
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Care and Coordination Agreement with
North Range Behavioral Health (NRBH). The purpose of this agreement is to ensure effective
communication and coordination between agencies at the local level. This agreement is a
stipulation of the contract between the Mental Health Centers and the Office of Behavioral
Health and only requires Director Signature. There will be no monetary exchange associated
with this agreement which will commence upon the date of final signature and will be in effect
no more than five years.
l do not recommend a Work Session. I recommend approval of this Agreement and approve
Department Director signature.

Care Coordination and Communication Agreement
Between
Weld County Department of Human Services, and
North Range Behavioral Health
SUBJECT: It is the intent on of North Range Behavioral Health and the Weld County
Department of Human Services to provide coordinated and quality care for mutual clients
and their families.
PURPOSE: To ensure effective communication and coordination between agencies at a local
level
�u. TERM OF AGREEMENT: This Agreement will commence upon the date of the final signature
and will be in effect no more than five (5) years. Changes in the Agreement may be made at
any time by mutual written consent of weld County Department of Human Services and
North Range Behavioral Health. Nothing in this Agreement shat substitute or represent a
change it any agency's legally mandated responsibilities
iv BACKGROUND AND BASIS FOR THE AGREEMENT: This Agreement is a stipulation of the
contract between the Mental Health Centers and the Office of Behavioral Health.
V PRINCIPLES OF THE AGREEMENT: The parties mutually agree to
1) provide written and verbal communication to each entity it a timely manner
2I Respond to emails in a timely manner
3! Create working relationships that are respectful and helpfu with the goal of meeting
client needs
In the event that either party is dissatisfied with the level of communication and/or
coordination of services, the following staff will be contacted to resolve issues and serve as
agency liaisons.
