RE: APPROVE ATTESTATION STATEMENT FOR COLLABORATIVE MANAGEMENT
INCENTIVE FUNDING AWARD FOR SFY 2017-2018 AND AUTHORIZE DIRECTOR OF
HUMAN SERVICES TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Attestation Statement for the
Collaborative Management Incentive Funding Award for SFY 2017-2018, from the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Department of Human Services, to the Colorado Department of Human
Services, Office of Children, Youth and Families, commencing August 8, 2017, with further terms
and conditions being as stated in said attestation statement, and
WHEREAS, after review, the Board deems it advisable to approve and accept said
attestation statement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Attestation Statement for the Collaborative Management Incentive
Funding Award for SFY 2017-2018, from the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Human Services, Office of Children, Youth and Families, to the Colorado Department of Human
Services, be and hereby is, approved and accepted.
BE IT FURTHER RESOLVED by the Board that the Director of Human Services be, and
hereby is, authorized to sign said attestation statement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 14th day of August, A.D., 2017, nunc pro tunc August 8, 2017.

RE: Weld County Department of Human Services' SFY2017-18
Collaborative Management Program (CMP) Attestation Statement
Please review and indicate if you would like a work session prior to placing this item on the Board's agenda.
Request Board Approval of the Departments' Request for Director signature on Attestation Statement
(attached). The Weld County Collaborative Management Program (CMP) MOU was accepted by the State and
an attestation statement is now required (attached); due to the State by September 1, 2017. The Weld County
CMP elected surplus distribution. The statement attests that the CMP will:
Track clients served in all target populations listed in the CMP site MOU;
Agree to not duplicating clients in the approved databases; and,
Comply with Operation Memorandum #OM-CW-2015-0009.
I do not recommend a Work Session. I recommend approval of Director signature and submission of the
attestation statement.

Re: Acceptance of Weld County Collaborative Management Program MOU for SFY 2017-18
Dear Director Griego,
The purpose of this letter is to communicate to you information about your county's participation in the
Collaborative Management Program (CMP), 24-1.9-101, also known as HB 04-1451.
The Weld County CMP Memorandum of Understanding (MOU) for SFY 2017-18 is accepted. Your county has
elected to participate in Surplus distribution, and did not request any waivers.
As specified in section IX of your counties MOU, Weld County needs to submit the attached attestation
statement with signatures prior to receiving the earned incentive funds.
The Colorado Department of Human Services, Division of Child Welfare appreciates Weld County's
continued commitment to your coordination and integration of services to children, youth and families in
your community.

In accepting the incentive funding award for Weld County, I, the County Commissioner(s) and/or County
Director, hereby attest that the Collaboarative Management Program wilt:
• Track client's served in all target populations listed in the CMP site MOU;
• Agree to not duplicating clients in the approved databases; and,
• Comply with Operation Memorandum #OM-CW-2015-0009.
